विषय.-प्रार्थना पत्र जेर धारा 13(3) जन्म एवं मृत्यु पंजीकरण अधिनियम, 1969 के अन्तर्गत मृत्यु तिथि को जन्म मृत्यु रजिस्टर में पंजीयन करने बारे।

यह कि श्री देवेन्द चौहान पुत्र स्व0 श्री आदी राम, ग्राम शिरगली, डाकघर देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) ने इस कार्यालय में अपनी बुआ श्रीमती कनावरी पत्नी स्व0 श्री आत्मा राम, ग्राम वासी शिरगली, डाकघर देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की मृत्यु को पंचायत मृत्यु रजिस्टर में6199 राजपत्र, हिमाचल प्रदेश, 26 दिसम्बर, 2016/5 पौष, 1938 पंजीकृत करने बारे आवेदन प्रस्तुत किया है। आवेदनकर्ता का कहना है कि उनकी बुआ की मृत्यु दिनांक 22-09-2012 को स्थान शिरगली मे हुई थी परन्तु वह अज्ञानतावश अपनी बुआ की मृत्यु को पंचायत में पंजीकृत न करवा सके, परन्तु अब वह इसको पंजीकृत करवाना चाहते हैं। आवेदनकर्ता ने आवेदन के साथ स्वयं का शपथ पत्र, अप्राप्यता प्रमाण पत्र, परिवार नकल एवम्‌ दो गवाहों के सत्यापित शपथ पत्र संलग्न किए हैं।

अतः इस इश्तहार राजपत्र द्वारा सर्वसाधारण को सूचित किया जाता है कि उक्त श्रीमती कनावरी पत्नी स्व0 श्री आत्मा राम, ग्राम वासी शिरगली, डाकघर देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की मृत्यु तिथि को मृत्यु रजिस्टर में पंजीकृृत करने से सम्बन्धित यदि किसी व्यक्ति विशेष को कोई उजर व एतराज हो तो वे दिनांक 06-01-2017 को समय प्रातः 11.00 बजे इस न्यायालय में उपस्थित होकर अपना एतराज प्रस्तुत कर सकते हैं। दिनांक 06-01-2017 के पश्चात्‌ कोई उजर एतराज पेश न होने की स्थिति में श्रीमती कनावरी पत्नी स्व0 श्री आत्मा राम, ग्राम वासी शिरगली, डाकघर देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की मृत्यु को मृत्यु रजिस्टर में पंजीकृत करने बारे सम्बन्धित पंचायत अधिकारी को आदेश पारित कर दिए जाएंगे।

आज दिनांक 05-12-2016 को मेरे हस्ताक्षर व मोहर अदालत सहित जारी हुआ।

यह कि श्री नरायण सिंह पुत्र स्व0 श्री मनी राम, ग्राम देहा, डाकघर देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) ने इस कार्यालय में अपने सपुत्र श्री राजन पुत्र नरायण सिंह, ग्राम वासी देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की जन्म तिथि को पंचायत रजिस्टर में पंजीकृत करने बारे आवेदन प्रस्तुत किया है। आवेदनकर्ता का कहना है कि उसके पुत्र का जन्म ग्राम देहा में घर पर ही हुआ है तथा उसकी जन्म तिथि 02-03-1990 है। परन्तु वह अज्ञानतावश अपने पुत्र की जन्म तिथि को पंचायत में पंजीकृत न करवा सके, परन्तु अब वह इसको पंजीकृत करवाना चाहते हैं। आवेदनकर्ता ने आवेदन के साथ स्वयं का शपथ पत्र, अप्राप्यता प्रमाण पत्र, आयु प्रमाण पत्र एवम्‌ दो गवाहों के सत्यापित शपथ पत्र संलग्न किए हैं।

अतः इश्तहार राजपत्र द्वारा सर्वसाधारण को सूचित किया जाता है कि उक्त श्री राजन सपुत्र श्री नरायण सिंह वासी ग्राम देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की जन्म तिथि को पंचायत रजिस्टर में पंजीकृृत करने से सम्बन्धित यदि किसी व्यक्ति विशेष को कोई उजर व एतराज हो तो वे दिनांक 06-01-2017 को समय प्रातः 11.00 बजे इस न्यायालय में उपस्थित होकर अपना एतराज प्रस्तुत कर सकते हैं। दिनांक 06-01-2017 के पश्चात्‌ कोई उजर/एतराज पेश न होने की स्थिति में श्री राजन सपुत्र श्री नरायण सिंह, वासी ग्राम देहा, उप-तहसील देहा, जिला शिमला (हि0 प्र0) की जन्म तिथि को पंचायत रजिस्टर में पंजीकृत करने बारे सम्बन्धित पंचायत अधिकारी को आदेश पारित कर दिए जाएंगे।

उनवान मुकद्दमा.--दरख्वास्त जेर धारा 13(3) जन्म एवम्‌ मृत्यु पंजीकरण अधिनियम, 1969 के अन्तर्गत। श्री बीनू पुत्र श्री रणवीर सिंह, निवासी गांव व डा0 डीम, तहसील जुब्बल, जिला शिमला, हि0 प्र0 ने अधोहस्ताक्षरी की अदालत में एक प्रार्थना-पत्र प्रस्तुत किया है जिसमें आग्रह किया है कि उसका नाम बीनू से बदलकर विवेक रखा जाए क्योंकि वर्तमान नाम ज्योतिष शास्त्र के अनुसार उसके लिए उपर्युक्त नहीं है इसलिए सभी सरकारी व गैर सरकारी दस्तावेजों में उसका नाम बीनू के स्थान पर विवेक रखा जावेगा। इसी संदर्भ में प्रार्थी ने समाचार पत्रों दिव्य हिमाचल में मिति 25-11-2016 व अमर उजाला में मिति 26-11-2016 को विज्ञापन बराए प्रकाशन दिए थे। जिसकी कतरनें प्रार्थना पत्र के साथ प्रस्तुत की गई हैं।

प्रार्थना पत्र में आगामी उचित कार्यवाही करने से पूर्व इस इश्तहार के माध्यम से सर्वसाधारण को सूचित किया जाता है कि यदि किसी व्यक्ति को भी उपर्युक्त प्रार्थी के नाम के बदलने पर कोई उजर व एतराज हो तो वह दिनांक 10-01-2017 को या इससे पूर्व अधोहस्ताक्षरी की अदालत में हाजिर होकर अपना लिखित व मौखिक एतराज प्रस्तुत करें, दीगर सूरत में यह समझा जाएगा कि प्रार्थी के नाम बदलने पर किसी को कोई आपत्ति नहीं है तथा ग्राम पंचायत पराली को प्रार्थी के नाम बदलने के आदेश पारित कर दिए जाएंगे।

उनवान मुकद्दमा.--दरख्वास्त जेर धारा 13(3) जन्म एवम्‌ मृत्यु पंजीकरण अधिनियम, 1969 के अन्तर्गत। इस कार्यालय में पूजा मान्टा पुत्री श्री पदम चन्द मान्टा, निवासी ग्राम कायना, डा0 कायना, तहसील जुब्बल, जिला शिमला, हि0 प्र0 का प्रार्थना-पत्र उप-मण्डल दण्डाधिकारी रोहडू़, जिला शिमला के माध्यम से प्राप्त हुआ है जिसमें प्रार्थिया ने निवेदन किया है कि प्रार्थिया की जन्म तिथि 12-04-1989 है जिसका पंजीकरण ग्राम पंचायत कोट कायना के अभिलेख में दर्ज नहीं है जिसके कारण जन्म प्रमाण-पत्र जारी नहीं हो पा रहा है इसलिए प्रार्थिया अब अपना नाम व जन्म तिथि सम्बन्धित पंचायत के रिकार्ड में दर्ज करवाना चाहती है।6201 राजपत्र, हिमाचल प्रदेश, 26 दिसम्बर, 2016/5 पौष, 1938 अतः इस इश्तहार के माध्यम से सर्वसाधारण को सूचित किया जाता है कि यदि किसी को भी उपरोक्त प्रार्थिया की जन्म तिथि का इन्द्राज ग्राम पंचायत कोट कायना में दर्ज करने बारे कोई भी उजर व एतराज हो तो वह दिनांक 10-01-2017 को प्रातः 10 बजे या इससे पूर्व किसी भी कार्य दिवस को अदालत हजा में हाजिर होकर अपना लिखित व मौखिक एतराज प्रस्तुत करे। यदि उक्त तारीख तक कोई भी उजर या एतराज प्रस्तुत न हुआ तो यह समझा जाएगा कि प्रार्थिया की जन्म तिथि पंजीकरण हेतु किसी को कोई आपत्ति नहीं है तथा ग्राम पंचायत कोट कायना को नाम व जन्म तिथि दर्ज करने के आदेश पारित किए जाएंगे।
