उपरोक्त मुकद्दमा उनवान वाला में प्रार्थी श्री कैला राम पुत्र स्व0 श्री ग्यारू राम, निवासी गांव तयामन, डा0 नम्होल, तहसील सदर, जिला बिलासपुर (हि0प्र0) ने इस अदालत में प्रार्थना-पत्र दिया है कि उसका पुत्र स्व0 श्री सुरेन्द्र कुमार पु़त्र श्री कैला राम, निवासी गांव तयामन, डा0 नम्होल, तहसील सदर, जिला बिलासपुर (हि0प्र0) की मृत्यु तिथि ग्राम पंचायत ध्याल के रिकार्ड में दर्ज नहीं है उसकी मृत्यु तिथि 24-09-2018 हैै। इसे दर्ज करने के आदेश किये जायें।

JLN) मेडिकल कॉलेज व हस्पताल चम्बा को सम्बोधित है उप-मण्डल दण्डाधिकारी चुराह के कार्यालय के माध्यम से इस कार्यालय मेें आवश्यक कार्यवाही हेतु प्राप्त हुआ है। जिसमें प्रार्थी ने निवेदन किया है कि उसकी पुत्री प्रियंका की जन्म तिथि 19-09-2018 है तथा उसका जन्म ग्राम पंचायत चरडा में हुआ है परन्तु भारत वर्ष में कहीं भी उसका नाम व जन्म तिथि जन्म रजिस्टर में दर्ज नहीं है इसलिए आवेदक ने अनुरोध किया है कि उसकीे पुत्री की जन्म तिथि ग्राम पंचायत चरडा, विकास खण्ड तीसा के जन्म रजिस्टर में पंजीकरण किया जाए।

अतः इस इश्तहार के माध्यम से सर्वसाधारण को सूचित किया जाता है कि यदि किसी भी व्यक्ति को प्रार्थी की पुत्री प्रियंका की जन्म तिथि का इन्द्राज ग्राम पंचायत चरडा के जन्म रजिस्टर में पंजीकरण करने बारा किसी प्रकार का कोई भी उजर व एतराज हो तो असालतन व वकालतन इस इश्तहार के प्रकाशन की तिथि उपरांत एक माह के भीतर किसी भी कार्य दिवस में सुबह 10.00 बजे से शाम 5.00 बजे तक अपना उजर एवं एतराज पेश कर सकते हैं। बाद तारीख किसी किस्म का उजर नहीं सुना जायेगा व प्रार्थी की पुत्री की जन्म तिथि को दर्ज करने के आदेश सचिव ग्राम पंचायत चरडा, तहसील चुराह को पारित कर दिए जायेंगे।
