CHANGE OF NAME

I, Surender Kumar (47) s/o Rasil Singh, r/o Village Roso, P.O. Sadhot, Tehsil Sarkaghat, District Mandi declare that my name is written as Surender Kumar Guleria in the School records of my son Aryan Guleria whereas other Government documents my name entered Surender Kumar. Therefore this should be corrected in the school records also and my wife name is written as Sapna whereas her correct name is Sapna Devi. All concerned please may note.
