यह दरख्वास्त श्रीमती राज कान्ता पुत्री स्व0 श्री सुरत राम, निवासी गांव व डा0 डन्सा, हालाबाद पत्नी स्व0 श्री रेवा नन्द, निवासी गांव चाकली, डा0 देवठी, तहसील रामपुर, जिला शिमला (हि0प्र0) ने इस कार्यालय में अपनी जन्म तिथि जन्म व मृत्यु पंजीकरण ग्राम पंचायत रचोली में दर्ज करवाने बारे आवेदन पत्र व ब्यान हल्फी दिया है। प्रार्थिया ने आवेदन किया है कि ज्ञान न होने के कारण वह अपनी जन्म तिथि का पंजीकरण ग्राम पंचायत डन्सा में दर्ज न कर सकी। प्रार्थिया की जन्म तिथि निम्न हैः-

अतः इस इश्तहार द्वारा सर्वसाधारण को सूचित किया जाता है कि ग्राम पंचायत डन्सा में प्रार्थिया की जन्म तिथि पंजीकरण बारे किसी भी व्यक्ति को किसी भी प्रकार का कोई उजर व एतराज हो तो वह दिनांक 17-02-2026 को प्रातः 11 बजे असालतन या वकालतन हाजिर अदालत आकर अपना उजर व ऐतराज पेश कर सकता है अन्यथा एकतरफा कार्यवाही अमल में लाई जावेगी। आज दिनांक 16-01-2026 को मेरे हस्ताक्षर व मोहर अदालत से जारी किया गया।

प्रार्थी श्री रवि कुमार पुत्र श्री धनबहादुर, निवासी कड़ियुथ, डा0 राजगढ़, ग्राम पंचायत शलाणा, तहसील राजगढ़, जिला सिरमौर (हि0प्र0) ने इस अदालत में अधीन धारा 13(3) जन्म एवं मृत्यु पंजीकरण अधिनियम, 1969 व हिमाचल प्रदेश जन्म एवं मृत्यु रजिस्ट्रेशन नियम 2003 की धारा 9(3) के अंर्तगत जिला रजिस्ट्रार (जन्म एवं मृत्यु) एवं मुख्य चिकित्सा अधिकारी, जिला सिरमौर स्थित नाहन के माध्यम से अपने पुत्र अभिनव कुमार की जन्म तिथि ग्राम पंचायत शलाणा में दर्ज करने हेतु प्रार्थना पत्र प्रस्तुत कर निवेदन किया है कि उसके पुत्र का जन्म घर पर हुआ है, परन्तु अज्ञानता के कारण निश्चित अवधि में जन्म की तिथि पंचायत के जन्म व मृत्यु रजिस्टर में दर्ज नहीं हो सकी । आवेदक के अनुसार उसके पुत्र अभिनव कुमार का जन्म दिनांक 23-09-2013 को हुआ है। सायल ने ग्राम पंचायत शलाणा के पंजीकरण पंजी में अपने पुत्र की जन्म तिथि दर्ज करवाने का अनुरोध किया हैै।

अतः इस इश्तहार द्वारा आम जनता को सूचित किया जाता है कि यदि किसी भी व्यक्ति को अभिनव कुमार पुत्र रवि कुमार की जन्म तिथि ग्राम पंचायत शलाणा में दर्ज करने बारे ऐतराज हो तो इस इश्तहार के प्रकाशन से 30 दिनों के भीतर किसी भी कार्यदिवस पर हमारे कार्यालय में असालतन या वकालतन हाजिर होकर ऐतराज प्रस्तुत करे। उक्त तारीख के बाद कोई भी ऐतराज मान्य नहीं होगा तथा रजिस्ट्रार (जन्म एवं मृत्यु) एवं सचिव ग्राम पंचायत शलाणा को जन्म तिथि दर्ज करने का आदेश पारित कर दिया जायेगा।
