No. 5176040 with Roll No. 256310654 of March 2005 issued from Board of Secondary Education, Madhya Pradesh , also known as Adhila Mariyam in the Discharge Certificate No. 2723 dated 21-10-2005 issuedfrom the Therbiyathul Islam Sabha, Kozhikode , Marriage Certificate dated 28-1-2006 issued from Kerala Nadvathul Mujahideen, Calicut and Adhila Mariyam Mohammad in the Residence Certificate No. 2279/07 dated 2-7-2007 issuedby Village Officer, Kasaragod Group is one and the same person . I have embraced Islam from Ezhava Hindu community as per the above Discharge Certificate . Hereafter I will be a member of Islam religion and will be known by the name Adhila Mariyam Mohammad Rafeeque only and will sign accordingly.
