Chirakkara, Thalassery Taluk, Kannur District, Pin-670 104 have changed the name of my minor daughter K. P. Hiba Nassar, holder of Birth Certificate with Registration No. 1188/II/92 dated 18-4-1992 issued from Thalassery Municipality extract from the Admission Register with Admission No. 2855 dated 7-6-2002, issued from Government Brennen Higher Secondary School, Thalassery as K. P. Hiba Nazer.
I also notify that my second minor daughter Fathima Zahma Nasser, K. P., holder of Birth Certificate with Registration No. 2111/II/96 dated, 20-5-1996 issued from Thalassery Municipality and also known as Fathima Nooha Nasser, K. P., in the extract of Admission Register with Admission No. 3558 dated 31-5-2006 issued from Government Brennen Higher Secondary School, Thalassery is one and the same person. I have changed her name to K. P. Fathima Nooha Nazer.
