1-4-2008 5 31-3-2009 സർക്കാർ നിശ്ചയിക്കുന്ന മറു കാലയളവിലേക്കോ ആരോഗ്യ വകുപ്പ് ഡയറക്‌ടറുടെ നിയന്ത്രണത്തിലുള ആശുപത്രികളിലേക്ക് ഭക്ഷ്യ- ഭക്ഷ്യേതര സാധനങ്ങൾ വിതരണം നടത്തുന്നതിനുള്ള മുദ്രവച്ച ദർഘാസുകൾ പ്രത്യേക കവറുകളിൽ 1-1-2008, 3 മണിക്ക് മുമ്പ് അതാതു സ്ഥാപനങ്ങളുടെ മേലധികാരികൾക്ക് കിട്ടിയിരിക്കേണ്ടതാണ് . ആരോഗ്യ വകുപ്പ് ഡയറക്‌ടർക്ക് നേരീട്ട് അയയ്ക്കുന്ന ദർഘാ സുകൾ സ്വീകരിക്കുന്നതല്ല.
Notice is hereby given inviting sealed tenders for the supply of in duplicate in separate covers Dietary Articles and Non-dietary Articles from 1-4-2008 to 31-3-2009 ог such other date that may be fixed by Government to the Medical Insitutions in the State under the Control ofDirector of Health Services and Public Health Laboratory , Thiruva- nanthapuram , so as to concerned not later than 3 p.m. on ist Tuesday of January 2008 ( 1-1-2008) .
Alist of such institutions can be had from the District Medical Officer of Health concerned.
ദർഘാസ് വ്യവസ്ഥകൾ , വിതരണം നടത്തേണ്ട സാധനങ്ങളുടെ പട്ടിക, രണ്ട് ദർഘാസ് ഫാരങ്ങൾ ദർഘാസിനോടൊപ്പം വയ്ക്കേണ്ട ഉടമ്പടികളുടെ മാതൃക മുതലായവ അതാതു സ്ഥാപനങ്ങളുടെ മേലധികാരി കളിൽ നിന്നും അവർ നിശ്ചയിക്കുന്ന ദർഘാസ് ഫാറത്തിന്റെ വില അടച്ച ഒരു ചെലാൻ രസീതു- വിൽപ്പന നികുതി അടച്ച വോദ്യ ചെലാൻ രസീതും 1-10-2007 -ലോ അതിനുശേഷമേ ലഭിച്ച അസ്സൽ സർട്ടിഫിക്കറും (കുടിക്കടപ്പുതം സഹിതം) ഹാജരാക്കി ആവശ്യപ്പെട്ടാൽ ലഭിക്കുന്നതാണ് . ส ณวาบ ഫാറം വാങ്ങുന്നതിനുള്ള തുക 10210- വൈദ്യ സഹായവും പൊതുജനാരോഗ്യവും 80 പല വരയും (ഭക്ഷ്യം ഭക്ഷ്യേതര വസ്‌തുക്കളുണ്ട് ദർഘാസ് ഫാറത്തിൻറെ വില അടയ്ക്കുന്നത് എന്ന ഹെഡ് 330 അക്കൗണ്ടിലും വിൽപ്പന നികുതി 102 സംസ്ഥാന വിൽപ്പന നികുതി ആക്ററ് .99 -നികുതി പിരിവുകൾ' എന്ന്ഹെഡ്‌ഡിലും അടയ്ക്കേണ്ടതാണ് . ഇങ്ങന ഒടുക്കുന്ന പണം യാതൊരു കാരണവശാലും വിരിച്ചു നൽകുന്നതല്ല .
ഓരോ ദർഘാസിനോടൊപ്പവും ടെണ്ടറുകളുടെ പത്തു ശതമാനത്തിൽ കുറയാത്ത നുകയ്ക്കുളള സോൾവൻസി സർട്ടിഫിക്കറ്റിന്റെ അസൗസ‌ലും ഒരു ശരിപ്പകർപ്പ കടം വീട്ടാൻ മുതലുള്ളത് വ്യാപ്ത‌ി എതുവരെയെന്നും എവിടെയാണ് (പ്രസ്തുത കരാറകരാറുകാരൻ താമസിക്കുന്ന തെന്നുമുളള അതാത് നിന്നുള്ള സർട്ടിഫിക്കാറും [ 18 (ബി) സ്റോർ പർച്ചേസ് റൂം ജനറൽ വിവരങ്ങൾ) (സർക്കാർ കത്ത് നമ്പർ 36489 / എ-3 / 2002 / ആ. കു.വ. തീയതി 9-5-2003 ) തഹശീൽദാരിൽ Copies ofconditions of tender schedule of Dietary Articles, tender form in duplicate and specimen copy ofthe form of agreement to be executed along with the tender etc. can be had from the respective institutions on production of two chalans one towards the cost of tender forms as prescribed by the officer- in-charge of the institutions concerned and the other towards the sales tax together with solvency certificate obtained on or after 1-1-2007 (supported by non encumbrance certificate) in Original . The head of account under which the cost of tender forms hat to be remitted to theGovern- ment Treasury is ' 0210-Medical and Public Health- 80-General -800-Ocher receipts- 88 Miscellancous" (remittance of the cost of tender form for dietary and non-dietary articles) and that of sales tax is "0040-sales tax 102 - receipts under State Sales Tax Act-99 -tax collections" . The amounts remitted are not refundable under any circumstances .
ഈ സർട്ടിഫിക്കറ് 3-10-2007 -ലോ അതിനു ശേഷമോ അതാതു റവന്യൂ അധികാരികളിൽ നിന്നും നിശ്ചിത ഫാറത്തിൽ വാങ്ങിയനും കുടിക്കട ഉണ്ടെങ്കിൽ അത് ഉൾപ്പെടുത്തിയതും ആയിരിക്കണം . വെന്യൂ അധികാരിയിൽ നിന്നുള്ള കുഴിക്കര സർട്ടി ഫിക്കറ്റിന്റെ അഭാവത്തിൽ അതാത് സ്‌ഥലാത്ത സബ് രജിസ്ട്രാറിൽ നിന്നും വാങ്ങി ഹാജരാക്കിയാലും മതി ."מכמונו
Each tender mustbe accompanied by solvency certificate in original , clearly indicating to what extent they are solvant from the Tahsildar of the Taluk where they reside along with their tenders as per clase 18(b) of General Conditions of Store Purchase Rules (Govt . Lt. No.36489/M3 /2002/ H&FWD dated 9-5-2003 ) and an attested copy in- corporating the encumbrance if any, in the prescribed form obtained on or after 1-10-2007 from the Competent Revenue Authorities.
കരാറുകാർ കരാർ സമർപ്പിക്കുമ്പോൾ 50 രൂപയുടെ മുദ്രപ്പത്രത്തിൽ സ്‌റേറാർ പർച്ചേസ " നിയമങ്ങളിലെ പൊതു വ്യവസ്ഥകളിൽ നിർദ ദേശിച്ചിട്ടുള്ള മ ഉടമ്പടി സമർപ്പിക്കേണ്ട മാണ് .
In the absence of the encumbrance certificate from the revenue authority, the same obtained from the Sub Registrar of the area concerned should be produced.

ബർഘാസുകൾ ആവശ്യമുളളവർ അതിൻറെ വിലയായ 400 രൂപയും വിൽപ്പന നികുതിയും കോപ്പിക്ക് 200 + നികുതിയും അടച്ച് ടി തീയതി പകൽ 3 മണിക്ക് മുമ്പായി ആഫീസിൽനിന്നും ദർഘാസുകളോടൊപ്പം 50 രൂപ മുദ്രപ്പത്രത്തിൽ ഒരു കറർ എഴുതി ഉള്ളടക്കം ചെയ്യേണ്ടതാണ് . വിലയുടെ 1 % നിരതദ്രവ്യം ടെണ്ടറിനോടൊപ്പം + എല്ലാ ദർഘാസുകൾക്കും ബാധകമായ നിയമങ്ങൾ ബാധകമാണ് . ഇതിനും ആഫീസിൽ നിന്നും കൂടുതൽ വിവരങ്ങൾ സമയങ്ങളിൽ പ്രവൃത്തി ലഭിക്കുന്നതാണ് .
