Sealed and competitive tenders are invited from the manufacturers under "Two Cover System " for the supply of Full Arm Disposable Gloves for the use of Various Veterinary Institutions of the Animal Husbandry Department for the year 2007-08.

First Cover (Cover A-Technical ) will have the following enclosures :

1. Specification of the items without rate .

2. Attested copy of manufacturing licence and Market standing certificate of the firm for a minimum period of three years from the licencing authority .

3. Attested copy of Sales Tax Clearance and Turnover Certificate obtained from Sales Tax Authorities.

4. Anffidavit in stamp paper worth Rs . 50 notarized by a Notary Public of either the place whree the person making the affidavit is residing or of Thiruvananthapuram .

5. copy of the test report to prove the quality of material used (test certificate issued by National

6. Copy of WHO , ISO Certification, if any :

7. A covering letter with name , address , Pin Code , Tel/ Fax No. , E-mail address and other details of the firm in which the list of enclosures are specified and addressed to the Director , Depart- ment of Animal Husbandry , Thiruvananthapuram. Pro forma for the above certificates are attached separately .

Second Cover ( Cover B- Financial) will have the following enclosures:

1. Quoted rate ( inclusive of Tax)

2. Original tes der form duly signed by authorized signatory .

3. Preliminary agreement in Kerala stamp paper worth Rs . 50 .

4. EMD. ( 1% of total cost of items quotedby means of Demand Draft in favour of the Director of Animal Husbandry , Kerala payable at Thiruvananthapuram.

Both covers (A & B ) mustbe effectively sealed and put in another cover , which is also sealed and submitted. Cover A & B willbe opened as per the schedule given below. Cover B of those tenderers who satisfy the above conditions in Cover Awill only be opened . The cover B of rejected tenderers will be returned without opening . Details of Full Arm Disposable Gloves to be supplied can be had from the tender form Government of Kerala Website www.kerala.gov.in and the department website zhdkerala.gov.in Intending tenderers may send the exact amount towards the cost of tender form . Ce ques , Postage Stamps , Postal Orders , Demand Drafts etc. will not be accepted as the cost of tender form nor the form be sent by Value Payable Post . Tenaer form can also be obtained from the Office of the Director of Animal Husbandry on payment of the cost in cash during office hours . Late tenders will not be considered under any circumstances . "Tender should be superscribed as tender for the supply of Equipments of Animal Treatment Kit and addressed to the Chief Veterinary Officer , District Veterinary Centre , DVC Campus , Bank Road, Kozhikode-673 001.

GENERAL CONDITIONS

1. The tender forms are neither transferable nor the cost is refundable under any circumstances .

2. A preliminary agreement in form given in the tender form is tobe executed in Kerala Stamp Paper of value of Rs . 50 .

3. An EMD at one percent of the cost of items tendered for shall accompany the tender by DD drawn in favour of the Chief Veterinary Officer , Kozhikode . This is subject to a minimum ofRs . 30. The firms who are exempted from furnishing EMD shouldbe enclosed the attested copy ofexemption order . EMD will not carry any interest.

4. The instruments supplied should have an on site warranty of two years and 5 years AMC .

5. Installation and training of officers should be done on firms cost .

6. Rates , Sales Tax and conditions of supply of articles should be clearly stated in respect of each item .

7. The articles should be supplied within two weeks from the date of supply order at their own cost .

8. Tenders should be submitted separately as technical bid and financial bid and superscribed so.

9. Technical bids will be opened first in the presence of such of the tenderers or their authorised representatives who may be present at that time and financial bids of those whose technical bids are found satisfactory will only be opened.

10. Due to any reason if the tender opening day is declared as a holiday the tender will be opened the next working day . on

without assigning any reason thereof. 11. The Cheif Veterinary Officer , Kozhikode reserves the right to accept or reject any tender 12. All the conditions applicable to Government teader willbe applicable to this tender also .
