On production of cash receipt Rs. 700+ VAT. The tenders shall make their own arrangements to remit the cash at the counter of FW wing in working hours to get cash receipt. Duplicate copy of the form will also be issued on payment of Rs. 400+ VAT. Copy No. VPP/IPO/DD/Postal stamps will acceptable towards the cost of tender form. The department is not responsible for postal delay if any in case of tender forms obtained by post. VAT will be charged extra.
Total cost—Rs. 10 lakhs.
EMD—Rs. 1%.

EMD should be in the form of Demand Draft addressed to Director of Health Services drawable at State Bank of Travancore, Trivandrum. An agreement in Kerala Government stamp paper worth Rs.50 may also be enclosed with the tenders. Late tenders will not be accepted.
The undersigned reserve the right to cancel the tender in unavoidable circumstances without stating any reason thereof. But the cost of tender forms will not be refunded at any rate.

Tender forms will be supplied from 14-12-2007 to 24-12-2007, 3 p.m. on production of Treasury Chelan for the sum of Rs. 400 + remitted under the Head of Account 0210—80—800—86 and Rs. 16 Sales Tax. Last date for receipt of the tender is on 26-12-2007, 11 a.m. and the tenders will be opened on the same day at 12.30 p.m. in the presence of the tenderers or their authorized representatives.
Late and incomplete tender will be summarily rejected. The tender should accompany a DD for the sum f o Rs. 2,500 or 1% of the quoted rate in favour of Superintendent, District Hospital, Kollam as EMD.

The Superintendent reserves the power to reject any tender without showing any reason thereof.
