ദർഘാസുകൾ സമർപ്പിക്കുന്ന കവറിന് പുറത്ത് ഫിസിയോ തെറാപ്പി ഉപകരണങ്ങൾ വിതരണം ചെയ്യുവാനുള്ള ദർഘാസ്" എന്ന് രേഖപ്പെടുത്തേണ്ടതും സൂപ്രണ്ട് , ഗവൺമെൻറ് ഹോമിയോപ്പതിക് മെഡിക്കൽ കോളേജ് ആശുപത്രി , കാരാപ്പറമ്പ് , കോഴിക്കോട് -10 എന്ന മേൽവിലാസത്തിൽ 12-2-2008 ന് പകൽ 11.30 മണിക്ക് മുമ്പായി ലഭിച്ചിരിക്കേണ്ടതാണ് . വൈകി ലഭിക്കുന്ന ദർഘാസുകൾ യാതൊരു കാരണവശാലും പരിഗണിക്കുന്നതല്ല . ലഭിച്ച ദര്ഘാസുകൾ അന്നേ ദിവസം 12മണിക്ക് ദർഘാസുകാരന്റെയോ അയാൾ ചുമതലപ്പെടുത്തിയ വ്യക്‌തിയുടെയോ സാന്നി ദ്ധ്യത്തിൽ തുറന്നു പരിശോധിക്കുന്നതും ഏററവും കുറഞ്ഞ നിരക്ക് രേഖപ്പെടുത്തിയതും ഗുണനിലവാര മുളളതുമായ ദർഘാസുകൾ അംഗീകരിക്കുന്നതുമാ‌ണ്.

ദർഘാസിൻറെ കൂടെ മൊത്തം തുകയുടെ ഒരു ശതമാനം വരുന്ന തുകയ്ക്കുള്ള ഡിമാൻറ് ഡ്രാഫ്റ് നിരതദ്രവ്യമായി അടക്കം ചെയ്യേണ്ടതാണ്.
Sealed competitive tenders are invited for the supply of Physiotherapy equipments for the use of Government Homoeopathic Medical College Hospital, Calicut- 10 . The list of equipments shall be obtained along with the tender form.
be The tender should superscribed "Tender for the supply of Physiotherapy equipments " and should be addressed to the Superintendent, Government Homoeo Pahic Medical College Hospital , Karaparamba , Calicut -10 . The last date for receipt of tender is at 11.30 a.m. on 12-2-2008. Late tenders will not be conssdered . The tenders will be opened at 12 Noon on the same day in presence of such of the tenderers or their authorized representatives who may be present at that time .
Estimate Amount Rs . 1.50 . Lakh .
ദർഘാസ് ഫോറം ആവശ്യമുളളവർ സൂപ്രണ്ട് , ഗവൺമെൻറ് ഹോമിയോപ്പതിക് മെഡിക്കൽ കോളേജ് ആശുപത്രി , കാരാപ്പറമ്പ്, കോഴിക്കോട° -10 എന്ന മേൽവിലാസത്തിൽ സ്വന്തം പേപ്പറിൽ അപേക്ഷി ០៣. ദർഘാസ ഫാറത്തിൻറ വില 450 രൂപ രണ്' . ഈ തുക യാതൊരു കാരണവശാലും തിരിച്ചു നൽകുന്നതല്ല. ദർഘാസ് ഫോറം കൈമാററം ചെയ്യാൻ പാടുള്ളതല്ല . ദർഘാസ് ഫോറം തപാൽവഴി ആവശ്യമുളളവർ ഫോറത്തിൻറെ വില കൂടാതെ 50 രൂപ തപാൽ ചാർജ്ജ് അയയ്ക്കേണ്ടതാണ് . ഡ്യൂപ്ലിക്കേറ് ഫോറം ആവശ്യമുള്ളവർ 225 രൂപ അധികം അയയക്കേ ണ്ടതാണ് . ദർഘാസ് ഫോറം 5-2-2008 വൈകുന്നേരം 3 മണിവരെ ആഫീസിൽ നിന്നും നേരിട്ടോ തപാൽ മുഖേനയോ ലഭിക്കുന്നതാണ്. തപാൽ രുഖേന ലഭിക്കണമെങ്കിൽ മണിയോർഡർ മാത്രമേ സ്വീകരിക്കു കയുളളൂ . വിതരണം ചെയ്യാനുള്°ദേശിക്കുന്ന ഉപകരണ ങ്ങളുടെ കാറ്റലോഗ് , സ്പെസിഫിക്കേഷൻ എന്നിവ ദർഘാസിൻറെ കൂടെ അടക്കം ചെയ്യേണ്ടതാണ് .
കൂടുതൽ വിവരങ്ങൾക്ക് പ്രവർത്തി ദിവസങ്ങളിൽ ആശുപത്രി ഓഫീസിൽ അന്വേഷിക്കേണ്ടതാണ് .
Intending tendierers may on application to the Superintendent, Govt . Homoeopathic Medical College Hospital, Calicut- 10 to obtain the requisite tender forms inwhich tender should be submitted. Application for tender forms should be accompanied by a cash remittance of Rs . 450. Which is the price fixed for a form which his not refundable . Tender forms if required through post an additional amount of Rs. 50 should remitted . Duplicate tender forms if required will be issuedat Rs . 225 per copy. Sale of tender forms by post will be closed at 3 p . m . on 5-2-2008 . Tender forms will be obtained from the hospital office on direct payment orby M.Ο.
