കവറിനുപുറത്ത് 15/2007-08 - سانه ഉപകരണങ്ങൾ നൽകുന്നതിന ' എന്ന് രേഖപ്പെടുത്തി ഡയറക്‌ടർ , ഇൻസ്ററിററ്റ്യൂട്ട് ഓഫ് അനിമൽ ഹെൽത്ത് ആൻറ് വെറ്ററിനറി ബയോളജിക്കൽസ് , പാലോട് തിരുവനന്തപുരം , കേരളം , لوله പിൻ -695 562 എന്ന വിലാസത്തിൽ നൽകേണ്ടതാകുന്നു . ദർഘാസുകൾ സ്വീകരിക്കുന്ന Om തീയതി 3-3-2009 പകൽ 11 മണിവരെയാണ് . താമസിച്ച് ലഭിക്കുന്ന ദർഘാസുകൾ സ്വീകരിക്കുന്നതല്ല . ദർഘാ സുകൾ 3-3-2008 പകൽ 12 മണിക്ക് ബന്‌ധപ്പെട്ട ദർഘാസുകാരനോ അംഗീകത ഏജൻറം പ്രസ്തുത സമയത്ത് ഹാജരുണ്ടെങ്കിൽ അവരുടെ സാന്നിദ്ധ്യത്തിൽ തുറക്കുന്നതാണ് .
Intending tenderers may on application to the Director, Institute of Animal Health and Veterinary Biologicals, Palode , Pacha P.O. , Thiruvanantha- puram, Pin-695 562 for obtaining the prescribed tender form on which the tender should be submitted . തിരുവനന്തപുരം ആനിമൽ ഹെൽത്ത് ആൻറ് വെറ്ററിനറി ബയോളജിക്കൽസിലേക്ക് ലിക്വിഡ് പാരാഫിൻ 500 മി . ലി . × 1050 എണ്ണം നൽകുന്നതിന് മൽസര സ്വഭാവമുള്ള മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചു കൊളളുന്നു .
വെററിനറി ബയോളജിക്കൽസ് , പാലോട് , പച്ച പോസ്റ് , തിരുവനന്തപുരം , കേരളം , പിൻ 695 552 എന്ന വിലാസത്തിൽ രേഖാമൂലം അപേക്ഷിക്കേണ്ടതാ കുന്നു . ഇങ്ങനെയുളള അപേക്ഷയോടൊപ്പം 43600 വിലയായി നിശ്ചയിച്ചിട്ടുള്ള 400 രൂപയും ( നാന്നും രൂപ മാത്രം ) അതിൻറ 12.5% നികുതിയും ചേർത്ത്പണമടച്ചതിന്റെ രസീതുകൂടി ഹാജരാക്കേണ്ടതാ കുന്നു. ഈ തുക മടക്കി നൽകുന്നതല്ല . ദർഘാസ് ഫോറം കൈമാററം ചെയ്യാൻ പാടുള്ളതല്ല . ദർഘാസ് ഫോറത്തിൻറെ വിൽപ്പന 1-3-2008 പകൽ 8 മണിക്ക് നിർത്തൽ ചെയ്യുന്നതാണ് . ദർഘാസ് ഫാറം തപാലിൽ ലഭിക്കണമെന്നുള്ളവർ ഫാറവിലയും നികുതിയും തപാൽ ചാർജും അടക്കം 500 ( രൂപ് അഞ്ഞൂറ് മാത്രം ) മണിയോർഡറായി മേറിപ്പറഞ്ഞ അയച്ചുതരേണ്ടതാണ് . ഫോറവിലയായി ചെക്കുകൾ , ഡിമാൻറ് (ഡ്രാഫ്‌റുകൾ , തപാൽ സ്‌ററാമ്പുകൾ മുതലായവ സ്വീകരിക്കുന്നതോ ഫോറം , വി പി.പി. ആയി അയയ്ക്കുന്നതോ അല്ല . ദർഘാസ് പ്രമാണങ്ങ ളുടെ ഡ്യൂപ്ലിക്കേറ് ആവശ്യമുള്ളവർ പ്രതി ഒന്നിന് 200 രൂപയും (ഇരുനും " രൂപ 20 ) 12.5% നികുതിയും അധികമായി നൽകേണ്ടതാണ് . വിലാസത്തിൽ

Sealed and competitive tenders are invited from the manufacturers under " Two Cover System " for the supply of instruments and equipments for the use of Livestock Products Inspecting Office , Ernakulam for the year 2007-08.
First Cover (Cover A- Technical ) will have the following enclosures :
1. Specification] of the items without rate and list of places whare it has been supplied and Installed.
2. Letter from original manufacturer ( in case of agents ) regarding assuarance about supply of spare parts, AMC conditions (Periodiod of visits in a year etc) , warranty conditions , year of warranty etc.
3. Attested copy of Sales Tax Clearance and Turnover Certificate obtained from Sales Tax Authorities.
4. An affidavit in stamp paper worth Rs . 50 notarized by a Notary Public of either the place where the person making the affidavit is residing or of Thiruvananthapuram.
5. Copy of ISO Certification , if any :

A covering letter with name, address , Pin Code, Tel /Fax No. , E-mail address and other details of the firm in which the list of enclosures are specified and addressed to the Director, Depart- ment of Animal Husbandry , Thiruvananthapuram. Pro forma for the above certificates ; are attached separately.
Second Cover (Cover B- Financial) will have the following enclosures :
1. Quoted rate ( inclusive of Tax)
2. Original tender form duly signed by authorized , signatory .
3. Preliminary agreement in Kerala stamp paper worth Rs . 50.
4. EMD . ( 1 % of total cost of items quoted by means of Demand Draft in favour of the Director of Animal Husbandry, Kerala payable at Thiruvananthapuram) .
Both covers (A & B ) mustbe effectively sealed and put in another cover , which is also sealed and submitted. Cover A & B will be opened as per the schedule given below . Cover B of those tenderers who satisfy the above conditions in Cover A will only be opened . The cover B of rejected tenderers will be returned without opening . Details of Instruments and equipments to be supplied can be had from the tender form Government of Kerala Website-www.kerala.gov.in and the department website ahdkerala.gov.in Intending tenderers may send the exact amount towards the cost of tender form . Cheques, Postage Stamps, Postal Orders , Demand Drafts etc. will not be accepted as the cost of tender form nor the form be sent by Value Payableable Post . Tender form can also be obtained from the Office of the Director of Animal Husbandry on payment of the cost in cash during office hours . Late tenders will not be considered under any circumstances . Tenders should be superscribed as "Tender No. 8/2007-08 for the supply of equipments for the use of Livestock products inspecting office , Ernakulam for the year 2007-08" .
Price of tender form :
Original- Rs . 2,000 + VAT @ 12.5%.
Duplicate-Rs . 1000+ VAT @ 12.5% .

Due date and time of receipt of tenders ( Cover A and B)-29-2-2008 , at p.m. Date and time of opening of Cover A- 1-3-2008 , 10.30 a. m.

SPECIAL CONDITIONS
1. The equiponents should have a minimum garantee period of two years from the date of installation .
2. The rate quoted should be inclusive of all taxes .
3. The rate should be quoted in Indian Currency, and payment willbe made in Indian Currency only.
4. The Security Deposit , furnished will be released only after satisfactory completion of the contract .
5. The rate should be for delivery F.O.R station of despatch rates offered by the firms are not acceptable and such tenders will be summarily rejected .
6. The quantities specified in the schedule are only approximate and they may vary according to actual need .
7. The succesful tenderers should complete all supplies with in the stipulated time stated in the supply orders. No extention of supply period will be granted.
8. The Director of Animal Husbandry reserves all right to cancel the tender without assigning any reason thereof.
9. The director of Animal Husbandry on his authority has the right to inspect the manufacturing unit.
10. Payment will be made only after satisfactory installation of the items .
11. The only jurisdiction for any dispute regarding this purchase willbe at Thiruvananthapura .
12. The Photocopies of the documents produced along with the tender should be attested by a Gazetted Officer .
13. If the date scheduled for opening of teader is declared as holiday, tenders willbe opened on the next working day at the same time. 14. Name and place to which the item is to be supplied . The Livestock Products Inspecting Office , Ernakulem .
15. The Director of Animal Husbandry reserves the right to delete or waive any of the above Special conditions.
