Original-Rs 400 Rs . 50 ( VAT) Duplicate-Rs . 200+ Rs . 25 (VAT ) Date of commencement of sale of tender form- 11-2-2008 .
Last date and time for sale of teader forms- 29-2-2008 , 11 a . m.
Due date and time for receipt of tender- 29-2-2008 at 3 p . m .
Date and time of opening of tender - 1-3-2008 , 11 a. m .
Address of officer from whom tender form³ are to be obtained and to whom tender are to be sent-District Medical Officer of Health , Kozhikode .
The cost of tender form will be accepted in cash or by money order only at office hours . An additional amount of Rs . 50 should be sent with the cost to the undersigned for those who wish to have the tender form by post . Required E.M.D. and agreement are to be furnished.
