Intendingtenderers may obtain the tender form onapplication in their letter heads addressed to the State Health Transport Officer , Directorate of Health Services, Thiruvananthapuram . On production of cash receipt Rs . 700+ VAT . The tenderers shall make their own arrangements to remit the cash at the counter FW wing in working hours to get cash receipt . Duplicate copy of the forms will also be issued on payment of Rs . 400 + VAT per copy . No. VPP / IPO/ DD/ Postal stamps will acceptable being the cost of tender forms . The Department is not responsible for postal delay , if any, in case of tender forms obtained by post VAT willbe charged extra .

An Agreement in Kerala Government Stamp Paper worth Rs. 50 may also be enclosed with the tenders . Late tenders will not be accepted .
