Sealed and competitivecompetitive tenders in the prescribed forms are invited for the supply of various items as specified in the schedule for the ZLFC workshop , PMR Department, Medical College , Kozhikode . The tender forms can be had from the office of the Zonal Limb Fitting Centre , Medical College Kozhikode direct on payment of Rs . 450 and by , registered post on payment of Rs . 480 by money order. The tender form should be superscribed as "Tender No. AC/ 361 / 2008 / ZLFC " and addressed to the Principal/Project Director, Zonal Limb Fitting Centre, Medical College, Kozhikode . The last date for receipt of tender is 3 p . m . on 20-10-2008 . Tenders will be opened at 11 a.m. on 21-10-2008 in the presence of such tenderers or their authorised representatives who may be present at that time. Delayed tenders will not be accepted.

Tender forms are not transferable . Sale of tender forms will be closed on 19-10-2003 at 2 p.m. Cheques , Drafts, Postage stamps etc. will not be accepted towards the cost of tender form nor will the forms be sent by V.P.P. Duplicate copy of tender form can be had on payment of Rs . 225. The date upto which the rates are to be remain firm is on 31-3-2009 .

IMPORTANT

The tenderers should be sent along with the tender an agreement executed and signed in Kerala Stamp Paper worth Rs . 50. EMD 1 % of the total cost of the items tendered for . Rate should be quoted against Indian Rupee payment only . Tenders without the required agreement and Earnest Money Deposit and tenders.not quoted against Indian Rupees are liable to be rejected . Specimen agreement is annexed to the prescribed tender form. Stamp will be -supplied to firm outside Kerala on request along with a remittance of Rs . 87 by money order .
