7. A covering letter with name, address , Pin Code , Tel /Fax No. , E-mail address and other details of the firm with the list of enclosure to be specified and address to the Director , Department of Animal Husbandry , Thiruvananthapuram.
Sealed competitive tenders are invited from the manufacturers under "Two cover system" for the supply of Veterinary Videoendoscope ( 2 Nos .) for the year 2008-09 .

Proforma for the above certificates to be attached separately .
Price of tender form :
1. Specification of the item with rate ( inclusive of taxes or any other charges ) and list of institu- tions where it has been supplied and installed .

Last date and time for sale of tender forms- 22-10-2008 at 4 p . m.
2. Letter from original manfacturer (in case of agents) regarding assuarance about supply of spare parts , AMC conditions (Period of visits in a year etc. ) , warranty conditions , years of warranty etc.
Due date and time for receipt of tenders- 23-10-2008 at 4 pm .
3. Copy of ISO Certification , if any,
Date and time for opening of tenders-24-10-2008- at 11.30 a . m .
4. Original tender form duly signed by authorized signatory.
5. Preliminary agreement in Kerala stamp paper worth Rs . 50.
Date upto which the rates are to be firm for acceptance-31-3-2009.

Details of instruments and equipments to be supplied can be had along with the tender form , Government of Kerala web site-www.kerala . gov.in and the department website-ahdkerala gov.in Intending tenderers may send the exact amount towards the cost of tender form and can have the tender from the office during office hours on remitting the amount in cash. Cheques , Postage Stamps, Postal Orders , Demand Drafts etc. will not be accepted as the cost of tender form nor the form be sent by Value Payable Post . Tender form can be obtained from the Office of the Director of Animal Husbandry on payment of the cost in cash during the office hours . Late tenders will not be considered under any circumstances . Tenders should be superscribed as " Tender No. 16/2008-09 for the supply of Veterinary Videoca- doscope (2 Nos. ) for the year 2008-09 ".
