5. ടെ≠റിൽ ഒരു കിലോഗ്രാം പോളിത്തീൻ ബാഗിന്റെയും, ഷീറ്റിന്റെയും വില എത്ര രൂപ വരുമെന്ന് അക്കത്തിലും അക്ഷരത്തിലും എഴുതിയിരിക്കണം.

6. സർക്കാരിന്റെ ടെ≠ർ വ്യവസ്ഥകൾ എല്ലാംതന്നെ ഇതിനും ബാധകമാണ്. ടെ≠ർ ദിവസം പൊതു അവധിയായി പ്രഖ്യാപിക്കുകയാണെങ്കിൽ തൊട്ടടുത്ത പ്രവർത്തി ദിവസം ഇതേ സമയത്തും വ്യവസ്ഥയിലും ടെ≠ർ നടത്തുന്നതാണ്.

മലപ്പുറം ജില്ലയിൽ താഴെ പറയുന്ന സ്ഥാപനങ്ങളിലേക്ക് 1-4-2009 മുതൽ 31-3-2010 വരെ ഭക്ഷ്യ ഭക്ഷ്യേതര സാധനങ്ങൾ വിതരണം ചെയ്യാൻ തയ്യാറുള്ളവരിൽ നിന്നും മുദ്രവച്ച ദർഘാസുകൾ (പ്രത്യേക കവറുകളിൽ ര≠ുപ്രതി വീതം) ക്ഷണിച്ചുകൊള്ളുന്നു.

ദർഘാസുകൾ അതാത് സ്ഥാപനങ്ങളിൽ 1-1-2009 ഉച്ചക്കു ശേഷം 3 മണിവരെ സ്വീകരിക്കുന്നതും അന്നേ ദിവസം 3 മണിക്ക് ദർഘാസുകൾ തുറക്കുന്നതുമായിരിക്കും. ദർഘാസുകൾ സമർപ്പിക്കുന്നതിനുള്ള ടെ≠ർ ഫോറവും മറ്റുപ്രമാണങ്ങളും അതാത് സ്ഥാപനങ്ങളിൽ നിന്നും 22-12-2008 മുതൽ 31-12-2008 പകൽ 5 മണിവരെ ലഭിക്കുന്നതായിരിക്കും. ആയതിലേക്കായി അവർ നി›യിക്കുന്ന ദർഘാസ് ഫോറത്തിന്റെ വില അടച്ച് ഒരു ചെലാൻ രസീതും 1-10-2008 ലോ അതിനുശേഷമോ ലഭിച്ച അസ്സൽ സോൾവൻസി സർട്ടിഫിക്കറ്റുകളും (കുടികിട പത്രം സഹിതം) ഹാജരാക്കേ≠താണ്.

ദർഘാസ് ഫോറം വാങ്ങുന്നതിനുള്ള തുക "0210-വൈദ്യ സഹായവും പൊതുജനാരോഗ്യവും , 80 ജനറൽ 800 മറ്റ് വര വുകൾ 88 പലവകയും" (ഭക്ഷ്യ ഭക്ഷ്യേതര സാധനങ്ങളുടെ ദർഘാസ് ഫോറത്തിന്റെ വില അടയ്ക്കുന്നത്) എന്ന ഹെഡ് ഓഫ് അക്കൗ≠ിലും വില്പന നികുതി "0040 വില്പന നികുതി 102 സംസ്ഥാന വില്പന നികുതി ആക്ട് 99-നികുതി പിരിവുകൾ" എന്ന ഹെഡിലും അടയ്ക്കേ≠താണ്. ഇങ്ങനെ ഒടുക്കുന്ന പണം യാതൊരുകാരണവശാലും തിരിച്ച് നൽകുന്നതല്ല.

Application for the tender form should be accompained by a cash remittance of Rs. 450 which is the price fixed for the form and which is not refundable under any circumstances. Tender forms are not transferable. Sale of tender forms will be closed at 1 p.m. on 9-1-2009. Cheque, postal orders etc. will not be accepted towards the cost of forms. Those who wish to purchase tender forms by post shall send an additional amount of Rs. 50 by money order to cover Postal Charges, Duplicate tender forms, if required will be issued at Rs. 225 per copy.

Intending tenderers should deposit an amount of 1% of the total cost quoted rate as Earnest Money Deposit by Demand Draft in favor of the Principal, Government Ayurveda College, Thiruvananthapuram, together with an agreement in Kerala Stamp paper worth Rs. 50. The rate quoted shall remain firm for acceptance till 31-7-2009. The Principal, Ayurveda College, Thiruvananthapuram, will have powers to postpone/cancel the tender.
