folle Sealed competitive tenders are invited for the supply of following equipments from original manufacturers or approved / authorised distributor for . the use of Obstetrics and Gynaecology Department to ESI Hospital , Parippally .

The firm should submit the following certificates along with the tenders :
( 1) The certificate that the quoted equipment have not been and are not being supplied to any other organization at a rate lower than being quoted here.
(2) A certificate about satisfactory performance of the equipment duly authenticated by other existing users of the equipment .
(3) Firms should also give a certificate that spares ofthe equipment quoted willbe made available at least for five years .
(4) Firm should undertake to enter into Annual Maintenance Contract atleast for five years after completion of warranty and accordingly quote the rates for Annual Maintenance Contract in the tender .
The firms has to submit two bids, technical bid and price bid each on the same day and but in seperate covers . The technical bid will be opened at first instance and decisions of Technical Evaluation Committee will be accepted regarding conformity with given specifications . As regards price bid eligible tender i.e. who qualifies the Technical Evaluation Committee evaluation at first instance will be opened. If any firm quotes more than one model ( i.c. higher and lower model ) . EMD of 1% separate for both quoted items should be submitted according to value of quoted item . In case only one EMD is followed/ enclosed for two models quoted , EMD willbe treated as for the lowest one .
The number and date of opening of the tender should be superscribed on the envelop containing the tender.
Cost of tender form:
Original-Rs . 450 .
Duplicate Rs . 225 .
Last date and time of sale of tender forms- 19-1-2009 , 2 p.m.
Last date and time of receipt of filled tender form-19-1-2009 , 4 p . m.
Date and time of opening of tender- 20-1-2009 , 11 a.m.
Acceptance and rejection of tender will be governed by Store Purchase Rules .
Tender forms can be obtained either direct from the Office of the Director of Insurance Medical Services on payment of its actual cost by cash /money order from 1-1-2009 or on post on request by paying Rs . 50 (Rupees Fifty only) additionally over and above the cost of tender forms . Filled tender form not subscribed to Store Purchase Rules and above conditions will be rejected summarily.
Tenders will be opened as specified below, as enjoined in the Store Purchase Rules .
The equipment tendered should confirm ISI specification . The prices quoted should be inclusive ofall taxes , duties and transportation charges to the place of delivery/specified by purchaser .
Notwithstanding the above , the undersigned reserves the right to accept or reject any/ all tenders without assigning any reason thereof but subject to Stores Purchase Rules . The purchase will be made strictly invoking Store Purchase Rules as applicable to any other purchase under Government Department and any omissions / oversight crept in the tender shall be treated as void if the clause/rules in Store Purchase Rules commit the same .
