SHORT TENDER NOTICE
No. 2049 (A) /A2/2008 /GHMCH. 9thJanuary 2009.
Sealed competitive tenders are invited to supply of Full Body Digital Color Doppler Ukra Sound Systemby buy back scheme for the use of Government Homoeo Medical College Hospital, Calicut-10.
be The tenders should superscribed "Tender for the supply of Full body Digital Color Doppler Ultra Sound System onby back schem" and addressed to the Superintendent , Government Homocopathy Medical College Hospital , Karaparamba , Calicut- 10. The Last date for receipt of tender is at 11.30 a.m. on 10-2-2009. Late tenders will not be considered , The tenders will be opened at 12 Noon on the same day in the presence of such of the tenderers or their authorized representatives who may be present at that time. Estimated amount Rs . 12 Lakhs .
Intending tenderers may on application to the Superintendent , Govt . Homoco Medical College Hospital, Karaparamba, Calicut- 10 will obtain the requisite tender forms on which tenders tender a cash should be submitted. Application for forms should be accompanied by remittance of Rs . 450 ( inclusive of VAT) which is the price fixed for a form which is not refundable. Tender forms if required through post an additional amount of Rs . 50 should be remitted . Duplicate tender forms if required will be issued at Rs . 225 per copy. Sale of tender forms will be closed at 3 p . m . on 9-2-2009 , (2-2-2009 by post). The tender forms will be obtained from the hospital office on direct payment or by Μ.Ο. only on working days .
A demand draft for an amount percentage of the total cost of items should be accompanied along with the tender Specification and catelogue also to be attached with tender. of one as EMD .
Any additional information required can be had from the hospital office of the undersigned during the working hours .
