Sealed tenders are invited for the repair of the following equipments by replacement of spares at State Public Health and Clinical Laboratory, Thiruvananthapuram.
Total Estimate Cost-Rs . 1,58.227 .
Intending tenderers may on written application to the Director, State Public Health and Clinical Laboratory, Thiruvananthapuram to obtain tender forms . Tender forms should be accompanied by a cash remittance which is the price fixed for a form and which is not refundable under any cirucmstances . Tender forms are not transferable.
Costof tender form:
OriginalRs . 400 + VAT.
Duplicate Rs . 200 + VAT.
Last date and time of sale of tender form- 25-2-2009 , 12 Noon .
Last date and time of receipt of tender form- 27-2-2009 , 12 Noon .
Date and time of opening of tenders- 2-3-2009 , 11 a. m.
Cheques and Bank Drafts etc. will not be accepted towards the cost of tender forms . Tender forms will be supplied to the firms by registered post. If the payment ismade by Money Order inclusive of postal charges of Rs . 25 for each form . Money Order willbe accepted only if received within 5 working days prior to the due date for submission of tenders . Duplicate tender form will be issued to those who have purchased of original tender forms . Tender should be accompanied by an agreement executed in Kerala Stamp Paper worth Rs . 50. and EMD 1% of the total cost of the items tendered . The rate offered should be inclusive of all taxes . Further details if any required can be had from this office during office hours.
Late tenders will not be accepted. Tender number and date should be superscribed on each envelope. Tender should be addressed to the Director, State Public Health and Clinical Laboratory, Thiruvananthapuram - 37 . The tenders will be opened in the presence of tenderers or their authorized representatives who are present at that time.

ദർഘാസ് ഫോറം ആവശ്യമുള്ളവർ അതിൻ്റെ വിലയും മണിഓർഡർ ആയോ നേരിട്ട് അടച്ചോ ഡയറക്ടർ , സ്റ്റേറ്റ് പബ്ളിക് ഹെൽത്ത് ആൻ്റ് ക്ളിനിക്കൽ ലബോറട്ടറി , തിരുവനന്തപുരം -37 എന്ന വിലാസത്തിൽ ഡയറക്ടർക്ക് രേഖാമൂലം അപേക്ഷിക്കാവുന്നതാണ് . തപാൽ മുഖേന ആവശ്യമുള്ളവർ മണിഓർഡർ ചാർജ്ജ് കൂടി ഓരോ ഫോറത്തിനും നൽകേണ്ടതാണ് . ദർഘാസ് സമർപ്പിക്കേണ്ട അവസാന തീയതിക്ക് 5 പ്രവൃത്തി ദിവസം മുമ്പെങ്കിലും ലഭിക്കുന്ന മണിയോർഡറുകൾ മാത്രമേ സ്വീകരിക്കുകയുള്ളൂ. ദർഘാസ് ഫോറം കൈമാറ്റം ചെയ്യാൻ പാടുള്ളതല്ല . ദർഘാസിന്റെ വില ചെക്കുകളായോ ഡ്രാഫ്റ്റുകളായോ പോസ്റ്റൽ ഓർഡറുകളായോ സ്വീകരിക്കുന്നതല്ല. അസ്സൽ ദർഘാസ് ഫോറം വാങ്ങുന്നവർക്ക് മാത്രമേ കോപ്പികൾ നൽക്കുകയുള്ളു. 50 രൂപ മുദ്രപ്പത്രത്തിൽ എഗ്രിമെൻ്റു കൂടി ടെണ്ടറിനോടൊപ്പം വയ്ക്കേണ്ടതാണ്. മൊത്തം തുകയുടെ 1 % ഇ.എം.ഡി. ആയി വയ്ക്കേണ്ടതാണ് . സാധനങ്ങളുടെ വിലയിൽ വിൽപ്പന നികുതി കൂടി ഉൾപ്പെടുത്തിയിട്ടുള്ള തുക കാണിച്ചിരിക്കേണ്ടതാണ്.

ദർഘാസുകൾ സംബന്ധിച്ച ഏതു തീരുമാനമെടുക്കുന്ന തിനും സ്റ്റേറ്റ് പബ്ലിക് ഹെൽത്ത് ഡയറക്ടർക്ക് പൂർണ്ണ അധികാരം ഉണ്ടായിരിക്കുന്നതാണ് .
Replacement Spares
1. Cold room
( a) Providing sealed split cooling system 2 ton (2Numbers) .
(b) Providing electric connection from the distribution to the outdoor unit . ( c) Providing isolator switch - 63 Amps ( 1 number).
(d) Providing starter/ Temperature Control -20 Amps (2Numbers).
(e) Providing temperature digital display with wiring.
(f) Providing exhaust fan with wiring.
Tender No. and date should be superscribed on such envelope and addressed to the Superintendent , Mental Health Centre, Thiruvananthapuram . Late tenders will not be accepted. Cost of tender form should not be returned under any circumstances . Tender should be accompanied by Earnest Money Deposit in the form of Demand Draft drawn in favour of " Superintendent, Mental Health Centre , Thiruvananthapuram ", which should not be less than 1 % ofthe price quoted. Duplicate tender forms will be issued, to those who purchased original . The undersigned has the right to reject any often without assigning reason thereof. Tender specification and more details can be had from the office on working days .
