TENDER NOTICE [3/2009-10]

Sealed competitive tenders are invited for the supply of Chemicals, Reagents , Biologicals , Kits Consumables etc. The tender should be superscribed " Tender No. 3 /2009-10-Supply of Chemicals, Reagents , Biologicals , Kits , and Consumables etc." and should be addressed to the Chief Disease Investigation Officer , Palode, Pacha P.O. , Pin-695 562 , Thiruvananthapuram District, Kerala . Last date for the receipt of the tender is 19-10-2009 at4 p . m. Late tenders will not be accepted. The tenders willbe opened at 11 a . m . on 20-10-2009 in the presence of the tenderers or their authorized representatives who inay be present at that time.
Estimate Cost of Rs . 1,30,000 .
Intending tenderers can apply to the Chief Disease Investigation Officer , Palode, Pacha P. O. , Pin-695 562, Thiruvananthapuram District for the prescribed tender form on which the tenders are to be submitted . Application for the tender form should be accompanied by a cash remittance of Rs . 400 (Rupees Four hundred only ) +VAT ( 12.5 % ) which is the price fixed for a set of tender form and which is not refundable . The tender forms are not transferable. Those who wish to get tender form by post may send Rs . 500 ( Rupees Five hundred only) MoneyOrder which include cost , taxes , postal charges etc. Sale of tender form will be closed at 12 noon on 16-10-2009 . Cheques , Postal Stamps, Demand Drafts etc. will not be accepted towards the cost oftender forms nor will the forms be sent as per V.P.P. Duplicate forms if required will be issued @ Rs . 200 Two hundred (Rupees only) + VAT extra per copy . Details of the tenders can be had from this office on all working days.
തിരുവനന്തപുരം ചീഫ് ഡിസീസ് ഇൻവെസ്ററി ഗേഷൻ ആഫീസിലേക്ക് കെമിക്കൽസ് , റീയേജൻറ്സ് കിററ്സ് ബറയാളജിക്കൽസ് , കൺസ്യൂമബിൾസ് എന്നീ ലബോറട്ടറി ഉപകരണങ്ങൾ നൽകുന്നതിനായി മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു . മൽസരാധിഷ്ഠിത ദർഘാസുകൾ " ദർഘാസ് നമ്പർ 3 /2009-10 സപ്ലൈ ഓഫ് കെമിക്കൽസ് , റീയേജൻറ്സ് ,യജൻറ്‌സ് , ബയോളജിക്കൽസ കിററ്സ് ആൻറ് കൺസ്യൂമബിൾസ് , ngama കവറിന് പുറത്ത് രേഖപ്പെടുത്തി ചീഫ് ഡിസീസ് ഇൻവെസ്റിഗേഷൻ ഓഫീസർ , പാലോട് , പച്ച പി.ഒ. , തിരുവനന്തപുരം , കോള , പിസ് -695 562 എന്ന വിലാസ ത്തിൽ വേണം സമർപ്പിക്കേണ്ടത് . ദർഘാസുകൾ സമർപ്പി ക്കേണ്ട അവസാന തീയതി 19-10-2009 വൈകുന്നേരം 4 മണി . ലഭിക്കുന്ന ദർഘാസുകൾ 20-10-2009 രാവിലെ 11 മണിക്ക് ദർഘാസ് നൽകിയവരുടെയനൽകിയവരുടെയോ അവർ ചുമതലപ്പെടുത്തിയവരുടെയോ സാന്നിദ്ധ്യത്തിൽ തുറന്ന് പരിശോധിക്കുന്നതാണ് . 2380 തുക 1,30,000 രൂപ .
നിർദ്ദിഷ" 9 ദർഘാസുകൾ ലഭിക്കുന്നതിന് ചീഫ് ഡിസീസ് ഇൻവെസ്ററിഗേഷൻ ഓഫീസർ , പാലോട് , പച്ച പോസ്ററ് , തിരുവനന്തപുരം , കേരളം , പിൻ -695 562 എന്ന വിലാസത്തിൽ ബന്‌ധപ്പെടേണ താണ് . നിർദിഷ്ട രൂപത്തിലുള്ള ദർഘാസുകളിൽ മാത്രമേ ദർഘാസ് സമർപ്പിക്കുംസമർപ്പിക്കുവാൻലഭിക്കുന്നതിനപ്പി പാടുളളു . ദർഘാസുകൾ ലഭിക്കുന്നതിന അപേക്ഷയോടൊപ്പം 400 രൂപ (നാനൂറ് രൂപ മാത്രം ) അതിന്റെ വാറ്റാ പ്രകാരമുളള നികുതിയും നൽകേണ്ടതാണ് . പ്രസ്തുത തൃക തിരികെ ലഭിക്കുന്നതല്ല . ഭർഘാസുകൾ കൈമാറാൻ പാടുള്ളതല്ല . ദർഘാസുകൾ തപാലിൽ ലഭിക്കണമെന്നുള്ളവർ 500 രൂപ ( അഞ്ഞൂറ് രൂപ മാത്രം ) ദർഘാസ് വില, വാറ്റ് നികുതി , തപാൽ ചാർജം ഇനങ്ങളിലായി മണിയോർഡര് അയയ‌ക്കേണ്ടതാണ് . ദർഘാസുകളുടെ വിൽപ്പന 16-10-2009 ഉച്ചയ്ക്ക് 12 മണിക്ക് അവസാനിപ്പിക്കുന്നതാണ് , ദർഘാസുകളുടെ + 83 . വില ചെക്കായോ പോസ്ററൽ ഓർഡറായോ ഡിമാൻറ് (ഡ്രാഫറായോ സ്വീകരിക്കുന്നതല്ല . ദർഘാസുകളുടെ പകർപ്പ് ആവശ്യമുളളവർ രൂപ 200 (ഇരുനൂറ് ) നികുതിയും അടച്ചാൽ ലഭിക്കുന്ന താണ്. ദർഘാസുകൾ സംബന്‌ധിച്ചുള്ള കൂടുതൽ വിവരങ്ങൾ എല്ലാ പ്രവൃത്തി ദിവസങ്ങളിലും ആഫീസ് സമയത്ത് ഈ ആഫീസിൽ നിന്നും അറിയാവുന്നതാണ് .
