കൊല്ലം ജില്ലയിൽ, കൊല്ലം ജില്ലാ ആശുപത്രിയിലെയും കൊട്ടാരക്കര താലൂക്ക് ആശുപത്രിയിലെയും ഓപ്പറേറ്റിംഗ് (ഠമസമഴമശ5) മൈക്രോസ്കോപ്പ് റിപ്പയർ ചെയ്യുന്നതിന് മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊളളുന്നു.

ദർഘാസുകൾ 11-3-2010, 3 മണിക്ക് മുമ്പായി കൊല്ലം ജില്ലാ മെഡിക്കലാഫീസർക്ക് സമർപ്പിക്കേ≠തും പ്രസ്തുത ദർഘാസുകൾ അന്നേദിവസം 3 മണിക്കുതന്നെ സമർപ്പിച്ച ആളിന്റെയോ അവർ ചുമതലപ്പെടുത്തിയിരിക്കുന്ന ആൾക്കാരു ടെയോ മുമ്പിൽവച്ച് തുറക്കുന്നതാണ്. ദർഘാസ് ഫോറത്തിന്റെ വിൽപ്പന 10-2-2010 മുതൽ 11-3-2010 പകൽ 1 മണിവരെ ഉ≠ായിരിക്കുന്നതാണ്. അടങ്കൽ തുക 55,000,രൂപ. ദർഘാസ് ഫോറം ഒറിജിനൽ തുകയുടെ 0.2 % മിനിമം 400 രൂപ, മാക്സിമം രൂപ 1,500+12.5 % വാറ്റ്. ഡ്യൂπിക്കേറ്റ് ഒറിജിനലിന്റെ 50 % +12.5 % വാറ്റ് എന്ന വിലയ്ക്ക് ഈ ഓഫീസിൽ നിന്നും എല്ലാ പ്രവൃത്തി ദിവസങ്ങളിലും ലഭിക്കുന്നതാണ്. ദർഘാസ് ഫോറത്തിന്റെ വില യാതൊരു കാരണവശാലും തിരിച്ച് ലഭിക്കുന്നതല്ല. ദർഘാസിൽ രേഖപ്പെടുത്തുന്ന തുകയുടെ 1 %-ൽ കുറയാത്ത തുകയ്ക്ക് ക്രോസ് ചെയ്ത ഡിമാന്റ ് ഡ്രാഫ്റ്റോ/ട്രഷറി സേവിംഗ്സ് ഡെപ്പോസിറ്റോ ജില്ലാ മെഡി ക്കൽ ഓഫീസറുടെ പേർക്ക് എടുത്ത നിരതദ്രവ്യമായും സ്റ്റോർ പർച്ചേസ് റൂളിൽ പറയുന്ന പ്രത്യേക ഫോറത്തിന്റെ 50 രൂപ മുദ്രപ്പത്രത്തിൽ എഗ്രിമെന്റും നിരതദ്രവ്യവും ഉള്ളടക്കം ചെയ്തി ട്ടില്ലാത്ത ദർഘാസുകൾ നിരസിക്കുന്നതിനുള്ള അധികാരം ജില്ലാ മെഡിക്കലാഫീസറിൽ നിക്ഷിപ്തമായിരിക്കുന്നതാണ്. മറ്റ് വിശദവിവരങ്ങൾ ഈ ആഫീസിൽ നിന്നും പ്രവർത്തി ദിവസങ്ങളിൽ 10 മണി മുതൽ 5 മണിവരെ അറിയാവുന്നതാണ്.

RE-TENDER NOTICE

No. C1-30125/2009. 21st January 2010.

Tenders are invited for the disposal of 6000 liters of used Hypo Solution in Medical College Hospital, Alappuzha.

Tender should be submitted in sealed envelopes addressed to Superintendent, Medical College Hospital, Alappuzha noting the Number C1-3025/09. Tender should be brought on 4-3-2010, 3 p. m. and will be received up to 5-3-2-2010, 3 p. m. Late tenders should not be accepted. Received tenders will be opened on 6-3-2010, 11 a. m. in the presence of tenderers or representatives of the tenders by the Superintendent, Medical College Hospital, Alappuzha. An agreement should be executed in Stamp Paper worth Rs. 50 and 11% of caution deposit. Each tender cost Rs. 400 for Original and Rs. 200 for Duplicate to be remitted in TR. 5.

Tenders will be cancelled by the Superintendent without any reason. The highest bidder has too removed the item within 7 days of the sanction after remitting the cost of the item and the admissible tax. The quality of the items should be examined before tendering.

Tender forms wanted persons or firms should collect the form from the Office of Medical College Hospital, Alappuzha which is situated in Vandanam before 3 p. m. of 4-3-2010.

Details regarding the tender will be obtained from the office on working days.

ആലപ്പുഴ മെഡിക്കൽ കോളേജ് ആശുപത്രിയിൽ സൂക്ഷിച്ചിരിക്കുന്ന 6000 ലിറററോളം വരുന്ന ഉപയോഗിച്ച ഹൈപ്പോ സൊല്യൂഷൻ വാങ്ങുവാൻ താല്പര്യമുളളവരിൽ നിന്നും മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊളളുന്നു.

ടെ≠ർ കവറിനു പുറത്ത് നമ്പർ സി1/3025/09 എന്നെഴുതി സൂപ്ര≠്, മെഡിക്കൽ കോളേജ് ആശുപത്രി, ആലപ്പുഴ എന്ന വിലാസത്തിൽ ദർഘാസ് സമർപ്പിക്കേ≠താണ്. 4-3-2010, 3 മണി വരെ ടെ≠റുകൾ വാങ്ങാവുന്നതാണ്. 5-3-2010, 3 മണി വരെ ടെ≠റുകൾ സമർപ്പിക്കേ≠താണ്. താമസിച്ചു കിട്ടുന്ന ടെ≠റുകൾ സ്വീകരിക്കുന്നതല്ല. 6-3-2010 രാവിലെ 11 മണിക്ക് ഹാജരുളള ടെ≠റുകാരുടെയോ അവരുടെ പ്രതിനിധികളുടെയോ സാന്നിദ്ധ്യത്തിൽ ടെ≠റുകൾ തുറക്കുന്ന തായിരിക്കും. ടെ≠റിനോടൊപ്പം 50 രൂപ മുദ്രപ്പത്രത്തിൽ കരാർ എഴുതി ഒപ്പിട്ട് തരേ≠തും മൊത്തം വിലയുടെ 1 % നിരതദ്രവ്യവും കെട്ടിവയ്ക്കേ≠തുമാണ്. ടെ≠ർ ഒന്നിന് 400 രൂപയും ഡ്യൂπിക്കേറ്റ് പകർപ്പിന് 200 രൂപയും ടാക്സ് ഇനത്തിൽ ഇരുപത്തിയഞ്ച് രൂപയും റ്റി. ആർ. 5-ൽ അടച്ച് രസീത് കാണിച്ച് ടെ≠ർ ഫോറം കൈപ്പറ്റേ≠താണ്.

ഏത് ടെ≠റുകളും നിരസിക്കുന്നതിനുളള അവകാശം ആശുപത്രി സൂപ്ര≠ിൽ നിക്ഷിപ്തമാണ്. ദർഘാസുകൾ അംഗീകരിച്ച് ഉത്തരവ് ലഭിച്ച് 7 ദിവസത്തിനുളളിൽ മൊത്തം വിലയും വിൽപ്പന നികുതിയും അടച്ച് സാധനങ്ങൾ നീക്കം ചെയ്യേ≠താണ്. ടെ≠റിന് മുമ്പുതന്നെ ഗുണനിലവാരം പരിശോധിച്ച് ഉറപ്പുവരുത്തേ≠താണ്. എല്ലാ ടെ≠റുകൾക്കും ബാധകമായ നിയമങ്ങൾ ഈ ടെ≠റിനും ബാധകമാണ്.

ടെ≠ർ ഫോറം ആവശ്യമുളളവർ നിലവിലുളള നിരക്കിൽ വിലയും നികുതിയും അടച്ച് 4-3-2010 പകൽ 3 മണിക്ക് മുമ്പായി ആലപ്പുഴ മെഡിക്കൽ കോളേജ് ആശുപത്രിയുടെ വ≠ാനത്ത് പ്രവർത്തിക്കുന്ന ആഫീസിൽ നിന്നും ടെ≠ർഫോറം വാങ്ങേ≠താണ്. കൂടുതൽ വിവരങ്ങൾ പ്രവർത്തി ദിവസങ്ങളിൽ ആഫീസിൽ നിന്നും ലഭിക്കുന്നതാണ്.

Sealed competitive and superscribed tenders are invited from reputed manufactures/dealers to preparation and display of "No Smoking" signboards installed as District Hospital, Kannur, General Hospital, Thalassery and District Medical Office of Health, Kannur.

Probable Amount of Cost—Rs. 36,000.

Cost of tender form :

Original—Rs. 300 + 12.5% VAT.

Duplicate—Rs. 150 + 12.5% VAT.

Date for sale of tender form—24-2-2010 to 1-3-2010.

Last date and time for receipt of tender form—

Intending tenders may on application to the District Medical Officer of Health, Kannur obtain requisite tender forms against cash remittance in which the tender to be submitted Cheques, Postal Orders, Demand Drafts etc. will not be accepted towards the cost of tender forms and the tender forms will not be sent by VPP.
