Sealed competitive tenders are invited for the Annual Maintenance Contract for the equipments (7 items) to Chief Disease Investigation Office, Palode, Pacha, Thiruvananthapuram.

The tender should be superscribed "Tender No. 13/2010 for the Annual Maintenance Contract (Deep Freezer—20, Deep Freezer—80, Refrigerator (Kelvinator), Refrigerator (Samsung), Refrigerator (Godrej), Refrigerator (LG), Air Conditioner (10 Nos.)" and should be addressed to the Chief Disease Investigation Officer, Palode, Pacha P. O., Pin-695 562, Thiruvananthapuram District, Kerala. Last date for the receipt of the tender is Intending tenderers can apply to the Chief Disease Investigation Officer, Palode, Pacha P. O., Pin-695 562, Thiruvananthapuram District for the prescribed tender form on which the tenders are to be submitted. Application for the tender form should be accompanied by a cash remittance of Rs. 300 + VAT (4%) and etc. which is the price fixed for a set of tender form and which is not refundable. The tender forms are not transferable. Those who wish to get tender form by Post may send Rs. 350. Money Order which include cost, taxes, postal charges etc. Sale of tender form will be closed at 12 noon on 14-12-2010. Cheques, Postal Stamps, Demand Drafts etc. will not be accepted towards the cost of tender form nor will the forms be send as V. P. P. Duplicate forms if required will be issued @ Rs. 150 (Rupees One hundred and fifty only) + VAT (4%) and Cess (1%) per copy. Details of the tenders can be had from this office on all working days.

14-12-2010 at 4 p. m. Late tenders will not be accepted. ദർഘാസുകൾ "ദർഘാസ് നമ്പർ 13/2010, Annual Maintenance Contract (Deep Freezer—20, Deep Freezer—80, Refrigerator (Kelvinator), Refrigerator (Samsung), Refrigerator (Godrej), Refrigerator (LG), Air Conditioner (10 Nos.)" എന്ന് കവറിന് പുറത്ത് രേഖപ്പെടുത്തി ചീഫ് ഡിസീസ് ഇൻവെസ്റ്റിഗേഷൻ ഓഫീസർ, പാലോട്, പച്ച പി. ഒ., തിരുവനന്തപുരം, കേരളം, പിൻ-695 562 എന്ന വിലാസത്തിൽ വേണം സമർപ്പിക്കുവാൻ. ദർഘാസുകൾ സമർപ്പിക്കേ≠ അവസാന തീയതി 14-12-2010 വൈകുന്നേരം 4 മണി. ലഭിക്കുന്ന ദർഘാസുകൾ 15-12-2010 പകൽ 10.30 മണിക്ക് ദർഘാസ് നൽകിയവരുടെയോ അവർ ചുമതലപ്പെടുത്തിയവരുടെയോ സാന്നിദ്ധ്യത്തിൽ തുറന്ന് പരിശോധിക്കുന്നതാണ്.

300 രൂപ (മുന്നൂറ് രൂപ മാത്രം) അതിന്റെ വാറ്റ് പ്രകാരമുള്ള നികുതിയും നൽകേ≠താണ്. പ്രസ്തുത തുക തിരികെ ലഭിക്കുന്നതല്ല. ദർഘാസുകൾ കൈമാറാൻ പാടുള്ളതല്ല. ദർഘാസുകൾ തപാലിൽ ലഭിക്കണമെന്നുള്ളവർ 350 രൂപ (മുന്നൂറ്റി അൻപത് രൂപ മാത്രം) ദർഘാസ് വില, വാറ്റ് നികുതി, തപാൽ ചാർ÷് ഇനങ്ങളിലായി മണിയോർഡർ അയയ്ക്കേ≠താണ്. ദർഘാസുകളുടെ വില്പന 14-12-2010 ഉച്ചയ്ക്ക് 12 മണിക്ക് അവസാനിപ്പിക്കുന്നതാണ്. ദർഘാസുകളുടെ വില ചെക്കായോ പോസ്റ്റൽ ഓർഡറായോ ഡിമാന്റ ് ഡ്രാഫ്റ്റായോ സ്വീകരിക്കുന്നതാണ്. ദർഘാസുകളുടെ പകർപ്പ് ആവശ്യമുള്ളവർ 150 രൂപ (നൂറ്റി അൻപത് രൂപ)+വാറ്റ് നികുതിയും അടച്ചാൽ ലഭിക്കുന്നതാണ്. ദർഘാസുകൾ സംബന്ധിച്ചുള്ള കൂടുതൽ വിവരങ്ങൾ എല്ലാ പ്രവൃത്തി ദിവസങ്ങളിലും ഓഫീസ് സമയത്ത് ഈ ഓഫീസിൽ നിന്നും അറിയാവുന്നതാണ്.
