Sealed competitive tenders are invited for the supply of Kraft Cover for all ESI Dispensaries in Kerala from original manufacturers or approved authorized distributors.

The firm should submit the authorization letter along with tender if it is supplied through authorized agent/distributor. Authorization letter if need be got confirmed from original dealer before finalizing tender. The number and date of opening of the tender should be super scribed on the envelope containing the tender.
Cost of tender form (including tax) :
Original—` 1,144.
Duplicate—` 595. Date and time of issue of tender—13-1-2011, 3 p.m.
Date and time of opening of tender—14-1-2011, 3 p.m.
Tenders received after the prescribed date and time will not be accepted. The maximum period admissible for delivery of the Kraft Cover should be 45-60 days since the date of supply order issued and to be supplied to 3 stores of ESI Department under PO.

Filled tender forms without EMD or inadequate EMD will be rejected summarily. Any doubt regarding these issue can be got clarified from Purchase Officer before filling the tenders. So incomplete tender or tender that violates Store Purchase Rules will be rejected. In short, tender opened on the specified date or rate quoted being lowest will not confer any right on the firms to get supply order. Tender forms will be available from the day once the notification/advertisement appearing in the news paper/Government website/Gazette as the case may be but upto 13-1-2011, 3 p. m. Tenders will be opened on 14-1-2011, 3 p. m. The prices quoted should be inclusive of all taxes, duties and transportation charges to the place of delivery/specified by the Director of Insurance Medical Services. The undersigned reserves the right to accept or reject the tender without assigning any reason as provided under Store Purchase Rules. The purchase will be made strictly in accordance with Store Purchase Rules as applicable to any other purchase under Government Department and shall be treated as void if the clause/rules in Store Purchase Rules committing the same. ഇൻഷ്വറൻസ് മെഡിക്കൽ സർവ്വീസസ് വകുപ്പിന്റെ കീഴിലുള്ള എല്ലാ ഇ.എസ്.എെ. ഡിസ്പെൻസറികളിലും എെ.പി.കൾക്ക് മരുന്ന് വിതരണം ചെയ്യുന്നതിനുവേ ി താഴെപ്പറയുന്ന അളവിലുള്ള ക്രാഫ്റ്റ് കവർ വിതരണം ചെയ്യുന്നതിന് മത്സരാടിസ്ഥാനത്തിലുള്ള സീൽ ചെയ്ത ടെ റുകൾ അംഗീകൃത വിതരണക്കാരിൽ നിന്നോ ഉല്പാദകരിൽ നിന്നോ ക്ഷണിച്ചുകൊള്ളുന്നു. ക്രമ എെറ്റം എണ്ണം അളവ് നമ്പർ 1 ക്രാഫ്റ്റ് കവർ 1000000 12 cm. 10 cm. 2 ക്രാഫ്റ്റ് കവർ 500000 11½ cm. 8½ cm അംഗീകൃത വിതരണക്കാരനോ ഏജന്റോ ആണ് വിതരണം ചെയ്യുന്നതെങ്കിൽ സ്ഥാപനത്തിന്റെ Authorization letter ടെ റിനോടൊപ്പം ഹാജരാക്കേ താണ്. ടെ ർ ഉൾക്കൊള്ളുന്ന കവറിന് പുറത്ത് നമ്പരും ടെന്റർ തുറക്കുന്ന തീയതിയും രേഖപ്പെടുത്തേ ടെ ർ ഫോറത്തിന്റെ വില (നികുതി അടക്കം) : അസ്സൽ—` 1,144. പകർപ്പ്—` 595. ടെ ർ സ്വീകരിക്കുന്ന അവസാന തീയതിയും സമയവും—13-1-2011, 3 മണി. ടെ ർ തുറക്കുന്ന തീയതിയും സമയവും— 14-1-2011, 3 മണി. നി›ിത തീയതിക്കും സമയത്തിനും ശേഷം ലഭിക്കുന്ന ടെ റുകൾ സ്വീകരിക്കുന്നതല്ല. സπൈ ഓർഡറിന്റെ തീയതിക്കു ശേഷമുള്ള 45 മുതൽ 60 വരെയുള്ള ദിവസങ്ങളാണ് കവർ വിതരണം ചെയ്യുന്നതിനുള്ള പരമാവധി കാലയളവ്. ഈ വകുപ്പിലെ മൂന്നു സ്റ്റോറുകളിലേക്കും വിതരണം ചെയ്യേ താണ്.
ഇൻഷ്വറൻസ് മെഡിക്കൽ സർവ്വീസസിന്റെ ഓഫീസിൽ നിന്നും നേരിട്ടോ അല്ലെങ്കിൽ 50 രൂപ മണിയോർഡർ മുഖേനയോ ടെ ർ ഫോമുകൾ സ്വീകരിക്കാവുന്നതാണ്. ഇൻഷുറൻസ് മെഡിക്കൽ സർവ്വീസസിന്റെ പേരിലെടുത്ത താണ്. പർച്ചേസിംഗ് ഓഫീസർ നി›യി 10,000 രൂപയുടെ ഡിമാന്റ് ഡ്രാഫ്റ്റ് ടെ ർ ഫോമിനോടൊപ്പം സമർപ്പിക്കേ ക്കുന്ന (1 മിനിമം) നിരതദ്രവ്യം ഇല്ലാത്തതോ മതിയായ തുകയ്ക്ക് അല്ലാത്തതോ ആയ ടെ ർ ഫോമുകൾ സ്വീകരിക്കു ന്നതല്ല.
സ്റ്റോർ പർച്ചേസ് റൂൾ/കേരള ഫിനാൻഷ്യൽ കോഡ് നിബന്ധനകൾ പാലിക്കാത്ത/അനുസൃതമല്ലാത്ത ടെ ർ ഫോമുകൾ സ്വീകരിക്കുന്നതല്ല. തുകകൾ എല്ലാ ടാക്സുകളും ഡ്യൂട്ടികളും ട്രാൻസ്പോർട്ടേഷൻ ചാർ÷ുകളും ഉൾപ്പെടു ന്നവയായിരിക്കും.
സ്റ്റോർ പർച്ചേസ് റൂൾ പ്രകാരം താഴെ ഒപ്പിട്ടിരിക്കുന്ന അധികാരിക്ക്/പർച്ചേസിംഗ് ഓഫീസർക്ക് ടെ ർ സ്വീകരിക്കു ന്നതിനോ തള്ളിക്കളയുന്നതിനോ ഉള്ള അധികാരം അന്തിമ മായിരിക്കും. പർച്ചേസ് നടത്തുന്നത് കർശനമായും സ്റ്റോർ പർച്ചേസ് റൂളിന് വിധേയമായിരിക്കും.
