Tender forms will be obtained from the Stores Officer, Government Medical Stores, Directorate of Health Services, Thiruvananthapuram-695 037, on payment of cash or Money Order. Those who want tender form by Order Money should sent Rs. 50 as postal charge for each tender. Money Order should reach this office before ten working days from the last date of sale of tenders. Postal Orders, Postal Stamps, Bank Drafts or Bank Cheques will not be accepted towards the cost of tender. "Special Terms & Conditions of the tender" shall be obtained with the tender form.

TENDER NOTICE

Sealed tenders are invited for the supply of Examination Tables—25 Nos. standard size with adjustable stainless head with following specification to Institute of Maternal & Child Health, Medical College, Calicut.
2830 mm Length × 610 mm Width × 760 mm Height 1.6 CR Sheet 32 Diameter × 1.6 Thickness Round tube four legs solid rubber bush. Two pieces movable lifting MS tubular construction head adjustable rust free finishing in IMCH, Medical College, Calicut-8 as noted below. Envelope containing tenders should be superscribed with tender numbers as indicated in the statement below and should be addressed to the Superintendent, IMCH, Medical College, Calicut-8, Kerala. Tenders must be submitted in the sealed covers. Tender will be opened on 18-1-2011, 12 noon. in the presence of such of the tenderers or their authorized persons who may be present at that time. Late tenders will not be accepted. Intending Tenderers may on application to the Superintendent, IMCH, Medical College, Calicut, Kerala will obtain the requisite tender forms in which the tenders should be submitted.

Application for tender forms should be accompanied by the cash remittance as noted above for a set of tender forms, which will not be refunded under any circumstances.
The equipment should have minimum two years warranty. All parts of the machine should be included in the warranty. The rate for CAMC/AMC for 5 years after the warranty period should be specified in the tender. Otherwise tender will not be considered. The tenders should undertake the responsibility for the intimate service even if any change in the dealership of the product supplied. The tender should be submitted the specification of the product with brochure of brand name.
The tender forms are not transferable. Cheque, Postage stamp, Draft, etc. will not be accepted towards the cost of tender forms. Tender forms will not be sent by VPP. Tender should be accompanied by EMD amounting to 1% (Rounded to next rupee) of the total cost of articles tendered for. The tenders not accompanied by sufficient EMD will not be entertained.
