SHORT TENDER NOTICE

No. S-2073/2010/AVCH. 10th December 2010.

Sealed competitive tenders are invited for Repair of Beds and Pillow with Good quality Rexin to the Government Ayurveda College Hospital, Kannur, Pariyaram.

Tenders should be superscribed "Repair of Beds and Pillow" addressed to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram. Last date of receipt of duly filled tender form is 3 p. m. on 27-1-2011. Sale of tender form will be closed at 4 p. m. on 24-1-2011. Late tenders will not be accepted under any circumstances. Tenders will be opened at 3.30 p. m. on 27-1-2011 in the presence of the tenderers or their authorized representatives who may be present at that time. The sample of rexine will be attached with tender.

Intending tenderers may on application to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram to obtain the required tender forms in which tenders should be submitted. Cost of Original tender form is ` 450 and Duplicate form is ` 225. Those who wished to purchase tender form by post should send additional amount of ` 50 as Postage Charge. Preliminary Agreement in Stamp Paper worth ` 100 and 1% of quoted amount as EMD should be enclosed with the quotation. The price of tender form is not refundable under any circumstances. Cheque, Postal Order, D.D. etc. will not be accepted towards the cost of form. Those who wish to purchase tender form by post shall obtain the forms early to avoid postal delay.

കണ്ണൂർ ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ് ആശുപത്രിയിലെ 150 കിടക്കകളും 75 പില്ലോ കവറുകളും മേൽത്തരം റക്സിൻ ഉപയോഗിച്ച് കവർ ചെയ്യുവാൻ താൽപര്യമുള്ളവരിൽ നിന്നും മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു.

ദർഘാസ് കവറിനു പുറത്ത് ''ബെഡും പില്ലോകളും റെക്സിൻ കവർ ചെയ്യുന്നതിനുളള ടെൻഡർ'' എന്ന് രേഖപ്പെടുത്തേ≠താണ്. ദർഘാസുകൾ ''സൂപ്ര≠് ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ് ആശുപത്രി, കണ്ണൂർ, പരിയാരം'' എന്ന പേരിൽ അയയ്ക്കേ≠താണ്. ടെൻഡറിന്റെ കൂടെ കവർ ചെയ്യുവാനുദ്ദേശിക്കുന്ന റെക്സിന്റെ സാമ്പിൾ ഉൾപ്പെടുത്തേ≠താണ്. ദർഘാസുകൾ സ്വീകരിക്കുന്ന അവസാന തീയതി 27-1-2011-ന് വൈകുന്നേരം 3 മണി. ടെൻഡർ ഫോറത്തിന്റെ വിൽപ്പന 24-1-2011-ന് 4 മണിക്ക് അവസാനിക്കുന്നതായിരിക്കും. ടെൻഡറുകൾ 27-1-2011 3.30-ന് അപ്പോൾ ഹാജരായിട്ടുളളവരുടെ സാന്നിദ്ധ്യത്തിൽ തുറക്കുന്നതായിരിക്കും. ഒറിജിനൽ ടെൻഡർ ഫോറത്തിന് 450 രൂപയും ഡ്യൂπിക്കേറ്റ് ഫോറത്തിന് 225 രൂപയും ആണ് വില. ടെൻഡർ ഫോറം തപാലിൽ ലഭിക്കേ≠വർ 50 രൂപ കൂടുതൽ മണിയോർഡറായി അയയ്ക്കേ≠താണ്. ചെക്ക്, പോസ്റ്റൽ ഓർഡർ ഡി. ഡി. തുടങ്ങിയവ ടെൻഡർ ഫോറത്തിന്റെ വിലയായി സ്വീകരിക്കുന്നതല്ല. പുരിപ്പിച്ച ടെൻഡർ ഫോറത്തിന്റെ കൂടെ രേഖപ്പെടുത്തിയ നിരക്കിന്റെ ആകെ തുകയുടെ 1% നിരതദ്രവ്യമായി നൽകേ≠ താണ്. 100 രൂപ മുദ്രപ്പത്രത്തിൽ ടെൻഡർ ഫോമിന്റെ കൂടെ നി›ിത മാതൃകയിൽ പ്രിലിമിനറി എഗ്രിമെന്റും ടെൻഡറിന്റെ കൂടെ വെയ്ക്കേ≠താണ് നിരതദ്രവ്യം ''സൂപ്ര≠,് ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ്് ആശുപത്രി, കണ്ണൂർ, പരിയാരം'' എന്ന പേരിൽ ഡി. ഡി. ആയി നൽകേ≠താണ്. ടെൻഡർ സംബന്ധിച്ച കൂടുതൽ വിവരങ്ങൾ ആശുപത്രി ആഫീസിൽ നിന്നും പ്രവൃത്തി സമയങ്ങളിൽ അറിയാവുന്നതാണ്.
