SHORT TENDER NOTICES

Sealed competitive tenders are invited for the supply of "Hospital Equipments" to the Government Ayurveda College Hospital, Kannur, Pariyaram for the year 2010-11. Estimate Cost is ` 3,87,000.

Tenders should be superscribed as "Tenders for the supply of Hospital Equipments" addressed to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram. Last date of receipt of duly filled tender form is 3 p.m. on 25- 1-2011. Sale of tender form will be closed at 4 p. m. on 22-1-2011. Late tenders will not be accepted under any circumstances. Tenders will be opened at 3.30 p.m. on 25-1-2011 in the presence of the tenderers or their authorized representatives, who may be present at that time. Brand name and pack size of the Hospital Equipments will be specified in the tender.

Intending tenderers may on application to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram to obtain the required tender forms in which tenders should be submitted. Cost of original tender form is ` 774 + 12.5% ST and duplicate form is

opened at 3.30 p.m. on 25-1-2011 in the presence of the tenderers or their authorized representatives, who may be present at that time. Brand name and pack size of the Hospital Equipments will be specified in the tender.

` 387 + 12.5% ST. Those who wished to purchase tender form by post should send additional amount of ` 50 as postage charge. The Preliminary Agreement in Stamp Paper worth ` 100 and 1% of quoted amount as EMD should be enclosed with the tender. The price of tender form is not refundable under any circumstances. Cheque, Postal Order, D.D. etc. will not be accepted towards the cost of form. Those who wish to purchase tender form by post shall obtain the forms early to avoid postal delay.

ദർഘാസ് കവറിന് പുറത്ത് "ആശുപത്രി ഉപകരണങ്ങൾ വിതരണം ചെയ്യുന്നതിനുള്ള ടെ≠ർ" എന്ന് രേഖപ്പെടുത്തേ≠താണ്. ദർഘാസുകൾ "സൂപ്ര≠്, ഗവൺമെന്റ ് ആയുർവേദ കോളേജ് ആശുപത്രി, കണ്ണൂർ, പരിയാരം" എന്ന പേരിൽ അയയ്ക്കേ≠താണ്. ദർഘാസുകൾ സ്വീകരിക്കുന്ന അവസാന തീയതി 25-1-2011 വൈകുന്നേരം 3 മണി. ടെ≠ർ ഫോറത്തിന്റെ വിൽപ്പന 22-1-2011 4 മണിക്ക് അവസാനിക്കുന്നതായിരിക്കും. ടെ≠റുകൾ 25-1-2011 3.30-ന് അപ്പോൾ ഹാജരായിട്ടുള്ളവരുടെ സാന്നിദ്ധ്യത്തിൽ തുറ ക്കുന്നതായിരിക്കും. ഒറിജിനൽ ടെ≠ർ ഫോറത്തിന്, ` 774 + 12.5% നികുതിയും ഡ്യൂπിക്കേറ്റ് ഫോറത്തിന്, ` 387 + 12.5% നികുതിയും, ആണ് വില. ടെ≠ർ ഫോറം തപാലിൽ ലഭിക്കേ≠വർ ` 50 കൂടുതൽ മണിയോർഡറായി അയയ്ക്കേ≠താണ്. ചെക്ക്, പോസ്റ്റൽ ഓർഡർ, ഡി.ഡി. തുടങ്ങിയവ ടെ≠ർ ഫോറത്തിന്റെ വിലയായി സ്വീകരിക്കുന്നതല്ല. പൂരിപ്പിച്ച ടെ≠ർ ഫോറത്തിന്റെ കൂടെ രേഖപ്പെടുത്തിയ നിരക്കിന്റെ ആകെ തുകയുടെ 1% നിരതദ്രവ്യമായി നൽകേ≠താണ്. ` 100 മുദ്രപത്രത്തിൽ ടെ≠ർ ഫോമിന്റെ നി›ിത മാതൃകയിൽ പ്രിലിമിനറി എഗ്രിമെന്റും ടെ≠റിന്റെ കൂടെ വെയ്ക്കേ≠താണ്. നിരതദ്രവ്യം "സൂപ്ര≠്, ഗവൺമെന്റ ് ആയുർവേദ കോളേജ് ആശുപത്രി, കണ്ണൂർ, പരിയാരം" എന്ന പേരിൽ ഡി.ഡി. ആയി നൽകേ≠താണ്. ടെ≠ർ സംബന്ധിച്ച കൂടുതൽ വിവരങ്ങൾ ആശുപത്രി ഓഫീസിൽ നിന്നും പ്രവർത്തി സമയങ്ങളിൽ അറിയാവുന്നതാണ്.
