SHORT TENDER NOTICE
No. S-783/2010/AVCH.
21st December 2010.
Sealed competitive tenders are invited for the supply of good quality Bedsheet and Pillow Cover as specified below to the Government Ayurveda College Hospital, Kannur, Pariyaram.
(1) Bed Sheet green with white boarder 60" × 90" (thick fine)—200 Nos.
(2) Pillow Cover Green 22" × 29" —200 Nos.
The tenders should be superscribed "Tenders for the supply of Bedsheet and Pillow Cover" addressed to the Super- intendent, Government Ayurveda College Hospital, Kannur, Pariyaram. Sale of Tender form will be started on 4-1-2011. Last date of receipt of duly filled tender form is 3 p. m. on 3-2-2011. Sale of Tender form will be closed at 4 P. M. on 31- 1-2011. Late tenders will not be accepted under any circumstances. Tenders will be opened at3.30 p.m. on 3-2-2011 in the presence of the tenderers or their authorized representatives who may be present at that time. The sample of rexine will be attached with tender. Intending tenderers may on application to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram to obtain the required tender forms in which tenders should be submitted. Estimate cost is ` 92,000. Cost of original tender form is ` 400 + 4% VAT and duplicate form is ` 200 + 4% VAT. Those who wished to purchase tender form by post should send additional amount of ` 50 as postage charge. Preliminary agreement in Stamp Paper worth ` 100 and 1% of quoted amount as EMD should be enclosed with the quotation. The price of tender form is not refundable under any circumstances. Cheque, Postal Order, D. D. etc. will not be accepted towards the cost of form. Those who wish to purchase tender form by post shall obtain the forms early to avoid postal delay.
കണ്ണൂർ ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ് ആശുപത്രി യിലേക്ക് 2010-11 വർഷത്തേക്ക് താഴെ പറയുന്ന അളവുകളി ലുള്ള ബെഡ്ഷീറ്റും പില്ലോകവറും വിതരണം ചെയ്യുവാൻ താൽപ്പര്യമുള്ളവരിൽനിന്നും മുദ്രവെച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു.
ബെഡ്ഷീറ്റ് — 60" × 90" — 200 എണ്ണം (പച്ച നിറത്തിൽ
വെള്ള ബോർഡർ).
പില്ലോകവർ — 22" × 29" — 200 എണ്ണം (പച്ച നിറം).
ദർഘാസ് കവറിന് പുറത്ത് "ബെഡ്ഷീറ്റും പില്ലോകവറും വിതരണം ചെയ്യുന്നതിനുള്ള ടെ≠ർ" എന്ന് രേഖപ്പെടുത്തേ≠ താണ്. ദർഘാസുകൾ "സൂപ്ര≠്, ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ് ആശുപത്രി, കണ്ണൂർ, പരിയാരം" എന്ന പേരിൽ അയക്കേ≠താണ്. ദർഘാസ് ഫോറത്തിന്റെ വിൽപ്പന 4-1-2011 മുതൽ ആരംഭിക്കുന്നതാണ്. ദർഘാസുകൾ സ്വീകരിക്കുന്ന അവസാന തീയതി 3-2-2011 വൈകുന്നേരം 3 മണി. ടെ≠ർ ഫോറത്തിന്റെ വിൽപ്പന 31-1-2011-ന് 4 മണിക്ക് അവസാനിക്കുന്നതായിരിക്കും. ടെ≠റുകൾ 3-2-2011-ന് 3.30-ന് അപ്പോൾ ഹാജരായിട്ടുള്ളവരുടെ സാന്നിദ്ധ്യത്തിൽ തുറക്കുന്നതായിരിക്കും. അടങ്കൽ തുക ` 92,000 ആണ്. ഒറിജിനൽ ദർഘാസ് ഫോറത്തിന് ` 400 + 4% വി. നി. യും, ഡ്യൂπിക്കേറ്റ് ഫോറത്തിന് ` 200 + 4% വി. നി. യും ആണ് വില. ടെ≠ർ ഫോറം തപാലിൽ ലഭിക്കേ≠വർ ` 50 കൂടുതൽ മണിയോർഡറായി അയക്കേ≠താണ്. ചെക്ക്, പോസ്റ്റൽ ഓർഡർ, ഡിമാന്റ ് ഡ്രാഫ്റ്റ് തുടങ്ങിയവ ടെ≠ർ ഫോറത്തിന്റെ വിലയായി സ്വീകരിക്കുന്നതല്ല. പൂരിപ്പിച്ച ടെ≠ർ ഫോറത്തിന്റെ കൂടെ രേഖപ്പെടുത്തിയ നിരക്കിന്റെ ആകെ തുകയുടെ ഒരു ശതമാനം നിരതദ്രവ്യമായി നൽകേ≠താണ്. ` 100 യുടെ മുദ്രപത്രത്തിൽ ടെ≠ർ ഫോമിന്റെ കൂടെ നി›ിത മാതൃകയിൽ പ്രിലിമിനറി എഗ്രിമെന്റും ടെ≠റിന്റെ കൂടെ വയ്ക്കേ≠താണ്. നിരതദ്രവ്യം "സൂപ്ര≠്, ഗവൺമെന്റ ് ആയൂർവേദ കോളേജ് ആശുപത്രി, കണ്ണൂർ, പരിയാരം" എന്ന പേരിൽ ഡി. ഡി. ആയി നൽകേ≠താണ്. ടെ≠ർ സംബന്ധിച്ച് കൂടുതൽ വിവരങ്ങൾ ആശുപത്രി ഓഫീസിൽനിന്നും പ്രവർത്തി സമയങ്ങളിൽ അറിയാവുന്നതാണ്.
