TENDER NOTICE
No.B-916(2)/2011.

Sealed tenders are invited for the supply of "Special MCR Chappals" tailored to patient needs, with adequate measurements to be taken in person by visiting the inpatient wards—150 pairs to Govt. Leprosy Hospital, Koratty.
Tenders should be superscribed tender for Special MCR Chappals and addressed to the Superintendent, Government Leprosy Hospital, Koratty. Late tenders will not be accepted. The tenders will be opened at 3 p. m. on 7-12-2011 in the presence of such of the tenderers or their authorized representatives who may be present at that time. The EMD will be 1% of the amount quoted.
Intending tenderers may on application to the Superintendent, Government Leprosy Hospital, Koratty obtain the requisite tender forms on which tenders should be submitted. Application for the tender form should be accompanied by a cash remittance of ` 450+4% VAT+1% Cess which is the price fixed for a form and which is not refundable under any circumstances. The tender forms are not transferable. Sale of tender forms will be closed at 3 p. m. on 6-12-2011. Cheques, Postage Stamps etc. will not be accepted towards the cost of forms nor will the forms be sent per V.P.P. Duplicate tender forms, if required will be issued at ` 200+4% VAT +1% Cess per copy.

Sealed competitive tenders are invited from the reputed firms/companies for the supply & installation of Biomedical incinerator at District Hospital, Palakkad.
Description of the Items—Biomedical incinerator with 100 Kg./hr. capacity Diesel based.
PAC—` 25 lakhs.
EMD—` 25,000.
Cost of tender form—` 3,954.

The tenders should be superscribed "Tender for installation of Biomedical incinerator" and should be addressed to the Superintendent, Dist. Hospital, Palakkad. The intending tenderers may obtain the requisite tender forms from the Office of the Superintendent from 10 a.m. to 3 p.m. on all working days on payment of required fee/cost from 18-11-2011 to 28-11-2011. The last date for receipt of duly filled in tender forms will be till 3 p.m. on 30-11-2011. The tenders received within the stipulated time will be opened at 3.30 p.m. on the same day in the presence of such of the tenderers or their authorized representatives who may be present at that time.
Intending tenderers may submit an application to the Superintendent, District Hospital, Palakkad to obtain the requisite tender forms on which the tenders should be submitted. The tender forms are not transferable Cheques, Postage Stamps etc. will not be accepted towards the cost of the forms nor will the form be sent through VPP.
The intending tenderers should enclosed EMD in lieu of tender by way of DD payable at Palakkad in favour of Superintendent, District Hospital, Palakkad along with the tender. The successful tenders should execute an agreement as in appendix XIV of store purchase manual and will remit security deposit of 5% of the total amount by way of D.D./Bank guarantee in favour of Superintendent, District Hospital, Palakkad.
Superintendent, District Hospital, Palakkad reserves the right to accept, reject or to cancel the tender received without assigning any reason thereof. Tenders shall satisfy special conditions appended to this notice and other general conditions on tender will also be applicable. Further details of the tender can be obtained from the Superintendent, during Office hours up to 28-11-2011 at 3 p.m.
SPECIAL CONDITIONS
1. The tenders shall invariably incorporate detailed technical specification approved by the Central Pollution Control Board.
2. The cost shall be for complete and all works in relation to the installation of the plant except civil works of the building.
3. The plan/requirement of building shall be specified along with the tender.
4. Power requirement details should be specified in detail.
5. Warranty period after installation shall be specified.
6. AMC/CMC rate chargeable after warranty period shall be required.
7. Average fuel consumption and water consumption shall be specified.
8. Any other requirement for the routine functioning like chemical, oil or any other items shall be specified.
