SHORT TENDER NOTICE
No. S1-6139/11/AVC.
16th November 2011.
Sealed competitive tenders are invited for the supply of the following items to the Collegiate Hospitals of Govt. Ayurveda College, Thiruvananthapuram for the year 2011-12 and for the supply up to one year from the date of fixing tender.
Tenders should be superscribed as "Supply of accessories for dental unit" and addressed to the Principal, Government Ayurveda College, Thiruvananthapuram. The last date of receipt of tenders will be at 3 p. m. on 20-12-2011. The late tenders will not be accepted. The tenders will be opened at 3.30 p. m. on 20-12-2011 in the presence of such of the tenderers or their authorized representatives who may be present at that time.
Estimate cost—` 36,000.
Intending tenderers may on application to the Principal, Government Ayurveda College, Thiruvananthapuram to obtain tender forms on which tenders should be submitted. Application for the tender form should be accompanied by a cash remittance of ` 312 (cost of tender form ` 300 + VAT ` 12) which is the price fixed for a form and which is not refundable under any circumstances. Tender forms are not transferable. Sale of tender forms will be closed at 1 p. m. on 20-12-2011. Cheques, Postal Orders etc. will not be accepted towards the cost of forms. Those who wish to purchase tender forms by post shall send an additional amount of ` 50 by money order to cover postal charges. Duplicate tender forms, if required will be issued at ` 156 (cost of tender form ` 150 + VAT ` 6) per copy.
Intending tenderers should deposit an amount of ` 360 as Earnest Money Deposit by Demand Draft in favour of the Principal, Government Ayurveda College, Thiruvananthapuram together with an agreement in Kerala Stamp Paper worth ` 100. The rate quoted shall remain firm for acceptance till one year from the date of acceptance. The Principal, Ayurveda College, Thiruvananthapuram will have powers to postpone/cancel the tender.
തിരുവനന്തപുരം ഗവൺമെന്റ ് ആയുർവേദ കോളേജ് ആശുപത്രി വിഭാഗങ്ങളിൽ ചുവടെ കാണിച്ചിരിക്കുന്ന സാധനങ്ങൾ വിതരണം ചെയ്യാൻ തയ്യാറുള്ളവരിൽനിന്നും മുദ്രവെച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു.
ഓരോ ദർഘാസുകളും ആയുർവേദ കോളേജ് പ്രിൻസിപ്പാ ളിന്റെ മേൽവിലാസത്തിൽ പ്രത്യേകം കവറിലാക്കി സീൽ ചെയ്ത് 20-12-2011 ഉച്ചയ്ക്ക് 3 മണിവരെ സമർപ്പിക്കാവുന്നതാണ്. അന്നുതന്നെ ഉച്ചയ്ക്കുശേഷം 3.30 മണിക്ക് ദർഘാസുകൾ നിയ മാനുസരണം തുറക്കുന്നതായിരിക്കും. യാതൊരു കാരണ വശാലും ദർഘാസ് ഫോറങ്ങൾ കൈമാറ്റം ചെയ്യാൻ പാടില്ലാത്ത താകുന്നു. ദർഘാസ് ഫോറങ്ങളുടെ വിൽപ്പന 20-12-2011 ഉച്ചയ്ക്ക് 1 മണിക്ക് അവസാനിക്കുന്നതാണ്. ഫോറങ്ങൾ തപാൽ മാർഗ്ഗം ആവശ്യമുള്ളവർ ദർഘാസ് ഫോറത്തിന്റെ വിലയേക്കാൾ ` 50 കൂടുതൽ മണിയോർഡർ ആയി അയയ്ക്കേ ≠താകുന്നു. ദർഘാസിൽ രേഖപ്പെടുത്തിയിരിക്കുന്ന വില നിരക്കുകൾ ദർഘാസ് അംഗീകരിച്ച് നൽകുന്ന തീയതി മുതൽ ഒരു വർഷം വരെയുള്ള കാലയളവിനുള്ളിൽ മാറ്റം വരുത്താൻ പാടില്ലാത്തതാകുന്നു. ഒരു ദർഘാസിന്റെ വില ` 312 ആണ്. (` 300 + വാറ്റ് ` 12). ഡ്യൂπിക്കേറ്റ് ഫോറത്തിന്റെ വില ` 156. അടങ്കൽ തുക ` 36,000.
പൂരിപ്പിച്ച ദർഘാസ് ഫോറത്തിനോടൊപ്പം നിരതദ്രവ്യ തുകയായി ` 360 തിരുവനന്തപുരം ആയുർവേദ കോളേജ് പ്രിൻസിപ്പാളിന്റെ പേരിൽ എടുത്ത ഡ്രാഫ്റ്റും ` 100-ന്റെ കേരള സംസ്ഥാന മുദ്രപത്രത്തിൽ രേഖപ്പെടുത്തിയ ഒരു എഗ്രിമെന്റും ഹാജരാക്കേ≠താണ്. ഏതൊരു ടെ≠റും കാരണം കാണിക്കാതെ നിരസിക്കാനോ മാറ്റിവയ്ക്കാനോ ഉള്ള അധികാരം തിരുവനന്തപുരം ആയുർവേദ കോളേജ് പ്രിൻസിപ്പാളിൽ നിക്ഷിപ്തമായിരിക്കും.

Application for the tender form should be accompanied by a cash remittance of ` 1,000 + 4% VAT, Total ` 1,040. Postal charges of ` 50 should be added if the forms needed by post. The price fixed for a tender form/set of forms is not refundable under any circumstances. The tender forms are not transferable. Sale of tender forms will be started at 10 a.m. on 1-12-2011 and will be closed at 3 p.m. on 15-12-2011. Demand Draft, Cheques, Postal Order etc. will not be accepted towards the cost of forms nor will the forms be sent by V.P.P. Duplicate tender form if requested will be issued at ` 500 + 4% VAT ; Total ` 520. A preliminary agreement as per Appendix XVII on Kerala Stamp Paper worth ` 100 together with an E.M.D. of 1% of the total amount quoted should be attached with the tender form by way of Demand Draft/Postal Order. Sealed tenders are invited for the supply of "Lab Equipments, Chemicals, Charts and Models for the Department of Rasasasthra and Bhaishajyakalpana of Government Ayurveda College, Kannur for the financial year 2011-12. Estimate cost is ` 5,00,000.
കണ്ണൂർ ഗവൺമെന്റ ് ആയുർവേദ കോളേജിലേക്ക് 2011-12 വർഷത്തേക്ക് രസശാസ്ത്ര ഡിപ്പാർട്ടുമെന്റിലേക്ക് രാസ വസ്തുക്കളും, ലാബ് ഉപകരണങ്ങളും ചാർട്ടുകളും മോഡലുകളും വിതരണം ചെയ്യുന്നതിനുള്ള ദർഘാസ് ക്ഷണിച്ചുകൊള്ളുന്നു. "പ്രിൻസിപ്പാൾ, ഗവൺമെന്റ ് ആയുർവേദ കോളേജ്, കണ്ണൂർ, പരിയാരം" എന്ന വിലാസ ത്തിലാണ് ദർഘാസ് അയയ്ക്കേ≠ത്. ദർഘാസ് കവറിന് പുറത്ത് "രസശാസ്ത്ര ഡിപ്പാർട്ടുമെന്റിലേക്ക് രാസവസ്തുക്കളും, ലാബ് ഉപകരണങ്ങളും ചാർട്ടുകളും മോഡലുകളും വിതരണം ചെയ്യുന്നതിനുള്ള ദർഘാസ" എന്ന് രേഖപ്പെടുത്തിയിരിക്കണം. 22-12-2011-ന് വൈകുന്നേരം 3 മണിക്ക് മുമ്പ് ദർഘാസ് ഓഫീസിൽ ലഭിച്ചിരിക്കണം. വൈകിക്കിട്ടുന്ന ദർഘാസുകൾ സ്വീകരിക്കുന്നതല്ല. ദർഘാസ് വാങ്ങിയവരുടെയോ അവരുടെ പ്രതിനിധികളുടെയോ സാന്നിദ്ധ്യത്തിൽ ദർഘാസുകൾ 22-12-2011-ന് വൈകുന്നേരം 3.30-ന് തുറക്കുന്നതായിരിക്കും. ദർഘാസ് ഫോറത്തിന്റെ വില ` 1,000 + 4% മൂല്യവർദ്ധിത നികുതി, ആകെ ` 1,040. ഫോറം തപാലിൽ ലഭിക്കുവാൻ വിലയോടൊപ്പം ` 50 അധികമായി മണിയോർഡർ അയയ്ക്കേ≠താണ്. ദർഘാസ് ഫോറത്തിന്റെ വില നി›ിതവും ഒരിക്കലും തിരിച്ചുനൽകാത്തതുമാണ്. ദർഘാസ് മറ്റൊരാളുടെ പേരിലേക്ക് മാറ്റാവുന്നതല്ല. ദർഘാസ് ഫോമുകളുടെ വിൽപ്പന ഈ ഓഫീസിൽ 1-12-2011-ന് രാവിലെ 10 മണി മുതൽ ആരംഭിക്കുന്നതും 15-12-2011-ന് വൈകുന്നേരം 3 മണിക്ക് അവസാനിക്കുന്നതുമാണ്. ചെക്ക്, പോസ്റ്റൽ ഓർഡർ, ഡിമാന്റ ് ഡ്രാഫ്റ്റ് തുടങ്ങിയവ സ്വീകരിക്കുന്നതല്ല. ഫോമുകൾ വി.പി.പി.ആയി അയയ്ക്കുന്നതല്ല. ഡ്യൂπിക്കേറ്റ് ഫോമുകൾ ആവശ്യമെങ്കിൽ `500-ന് (` 500 + 4% മൂല്യവർദ്ധിത നികുതി ആകെ ` 520) ലഭിക്കുന്നതാണ്. പൂരിപ്പിച്ച ദർഘാസി നോടൊപ്പം രേഖപ്പെടുത്തിയ മൊത്തം തുകയുടെ 1% നിരതദ്രവ്യവും ` 100-ന്റെ മുദ്രപത്രത്തിൽ പ്രിലിമിനറി എഗ്രിമെന്റും സമർപ്പിക്കേ≠താണ്. കൂടുതൽ വിവരങ്ങൾ കോളേജ് ഓഫീസിൽനിന്നും പ്രവർത്തി സമയങ്ങളിൽ അറിയാവു ന്നതാണ്. അടങ്കൽ തുക ` 5 ലക്ഷം.
