Sealed tenders are invited for the supply of the following item to the Regional Institute of Ophthalmology, Government Ophthalmic Hospital, Thiruvananthapuram.
Tender No.—A2-599/11/RIO.
Name of Equipment—Direct Opthalmoscope (15 Nos. X ` 15,000 per unit)
Cost of Tender forms :
Original—` 450 + VAT.
Duplicate—` 250 + VAT.
The tender should be superscribed "due at 11 a.m. on 20-12-2011" and addressed to the Director, Regional Institute of Ophthalmology, Government Opthalmic Hospital, Red Cross Road, Thiruvananthapuram-695 037.
Last date of receipt of tender is at 11 a. m. on 20-12-2011. Late tenders will not be accepted. The tender will be opened at 11.30 a. m. on 20-12-2011 in the presence of the tenderers or their authorized representatives who may present at that time.
Intending tenderers may on application to the Director, Regional Institute of Ophthalmology, Red Cross Road, Thiruvananthapuram-695 037, obtain the requisite tender form on which tenders should be submitted. Application for the tender form should be accompanied by a cash remittance of rupees noted above which is price fixed for a form and which is not refundable under any circumstances. The tender forms are not transferable. Sale of tender forms will be closed 3 p.m. on 19-12-2011. Cheque, postage stamp etc. will not be accepted towards the cost of tender forms, nor will the forms be sent per V.V.P. Duplicate tender forms, if required, will be issued at rupees noted above.
Detailed specifications and special conditions will be available with the tender form.
Further details if any required will have from the office of the undersigned in office hours on all working days.
The undersigned reserves the right to split the order among two or more tenders if found necessary and also to reject any of or all the tenders without assigning any reason.

7. Earnest Money Deposits (One Percentage) of the total cost of the articles quoted for, can be remitted by way of crossed Demand Drafts drawn in favour of the Principal, Medical College, Thiruvananthapuram.
8. Firms will be exempted from furnishing Earnest Money Deposit if they had produced the duly attested copies of relevant certificate along with their tenders.
9. Tenders Unsealed, Unsigned or submitted without superscription as specified are liable to be rejected.
10. Tenders submitted with insufficient Earnest Money Deposit will not be entertained.
11. Belated tenders will not be considered under any circumstance.
12. For Medicines, Surgicals and Materials, samples should be submitted along with the tender. Tenders without samples will not be considered for selection.
13. Tenders should bear the signature and full postal address of the tenderers whereever necessary.
14. The undersigned reserves the right to reject any tender wholly or partially without any prior information to the tenders.
15. Specifications of the items are given in the list.
16. Negotiation will be made only with the firm offering lowest price whose specification confirm to the requirements in the tender notification.
17. Tender will be opened in the presence of such of the tenderers or his authorized representatives who may present at the time.

SCHEDULE
1. The period for which the rates to be in firm—
One year.
2. The rates should be inc1usive of all taxes, otherwise tender will be rejected without any notice and for free delivery at the department concerned.
3. Advance payments conditions will not be considered.
4. Tender forms are neither transferable nor the cost of it will be refundable under any circumstances.
7. Earnest Money Deposits (one Percentage) of the total cost of the articles quoted for, can be remitted by way of crossed Demand Drafts drawn in favour of the Principal, Medical College, Thiruvananthapuram.
8. Firms will be exempted from furnishing Earnest Money Deposit if they had produced the duly attested copies of relevant certificate along with their tenders.
9. Tenders Unsealed, Unsigned or submitted without supercription as specified are liable to be rejected.
10. Tenders submitted with insufficient Earnest Money Deposit will not be entertained.
11. Belated tenders will not be considered under any circumstances.
12. For Medicines, Surgicals and Materials, Samples should be submitted along with the tender. Tenders without samples will not be considered for selection.
13. Tenders should bear the signature and full Postal address of the tenderers where ever necessary.
14. The undersigned reserves the right to reject any tender wholly or partially without any prior information to the tenders.

SPECIFICATION
1. Polysomnograh system should include a system that records and displays Physiological Parameters with at least 32 channels having following input channels. EEG, EOG, Chin EMG, Leg EMG, ECG., Nasal/Oral Airflow, Respiratory effort, CPAP Pressure, Abdominal Movements, Thoracic Movements, Limb Movements, Body Position, Snoring, Pulse Oximetry and Pulse transit time (PTT).
2. Should have at least 32 programmable amplifier channels with facilities for 8 additional DC channels for external peripherals like capnography pH etc. 3. Should have both pressure and thermistor based flow.
4. Should have RIP belts for thoracic and abdominal effort.
5. Should have integrated pulse oximeter.
6. Should have on screen impedence check and self calibration.
7. RDI Should be available during acquisition of data.
8. Should have adjustable gains and filters.
9. Should have user defined montages.
10. Should have provison for availability of all raw data for review in multiple user defined format.
11. Should have automatic sleep staging for adult and pediatric with manual override, respiratory analysis PLMS analysis, Neurological events.
12. Should have fully synchronized and integrated patient video and Audio Recording and monitoring with facility for video editing.
13. Should have ability for re-referencing, re-mortgaging and re-filtering at any time during a study or after the study has been recorded.
14. Should be provided with universal titration device with capability of titrating OSA, COPD, OHS and CSR patients.
15. Should have LAN interface for data communication
to P. C.
16. Should include patient interface head box, all sensors and electrodes kits, cables and software and standard accessories.
17. System should have option of scoring sleep and
other events as per AASM guidelines.
18. Manufacturer should have a wide National network and office with complete technical and application backup capability.
19. Should be US FDA certified.
20. 2 years or more onsite service and replacement warrenty and AMC for 5 years after warrenty period.
