Sealed and competitive tenders are invited from intending tenderer for the supply of Hospital equipment to Government Homoeopathic Medical College Hospital, Thiruvananthapuram.

Estimate cost—` 3.5 lakh.

E.M.D.—1% of the total cost.

By Direct— ` 700 + VAT.

By Money Order—` 750 + VAT.

തിരുവനന്തപുരം എെരാണിമുട്ടം സർക്കാർ ഹോമിയോപ്പതിക് മെഡിക്കൽ കോളേജ് ആശുപത്രിയിലേക്ക് ആശുപത്രി ഉപകരണങ്ങൾ വിതരണം ചെയ്യുന്നതിന് അംഗീകൃത വിതരണക്കാരിൽ നിന്നും മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു.

അടങ്കൽ തുക—` 3.5 ലക്ഷം. നിരതദ്രവ്യം—മൊത്തം തുകയുടെ 1%. ദർഘാസ് ഫോറത്തിന്റെ വില:

നേരിട്ട്—` 700 + വാറ്റ്. മണിയോർഡർ മുഖേന—` 750 + വാറ്റ്.

ടെ≠ർ ഫോറവും സാധനസാമഗ്രികളുടെ പട്ടികയും മറ്റ് വ്യവസ്ഥകളും തിരുവനന്തപുരം ഗവ.ഹോമിയോപ്പതിക് മെഡിക്കൽ കോളേജ് ആശുപത്രി സൂപ്ര≠ിന്റെ ഓഫീസിൽ നിന്നും ലഭിക്കുന്നതാണ്.

SHORT TENDER NOTICE

No. 2380/A2/11/GHMCH. 11th September 2011.

Competitive sealed tenders are invited to supply Lab equipments for the use of Government Homoeo Medical College Hospital, Karaparamba, Calicut-10. The list of equipments will be available along with the tender form.

Estimate cost—` 2,25,000.

E.M.D.—1% of the total cost.

Tender shall be superscribed " Tender for supply of Lab equipments" and addressed to the Superintendent, Government Homoeopathic Medical College Hospital, Karaparamba, Calicut -10. The last date for receipt of tender is 3-1-2012 at 11 a. m. Tenders will be opened at 11. 30 a. m. on the same day in the presence of tenderers or their authorized representatives who may be present at that time.

Those who require tender forms should apply to the Superintendent, Government Homoeo Medicial College Hospital, Karaparamba, Calicut-10. Application for tender form should be accompained by a cash remittance of ` 450+Kerala VAT which is the price fixed for a form and which is not refundable under any circumstances. Sale of tender form will be closed at 3 p.m. on 2-1-2012 (24-12-2011 by post). The tender forms will be obtained from the hospital office on direct payment or by Money Order only on working days. Those who want tender forms by post should send an additional amount of ` 50 towards postage charges. Duplicate forms, if required will be issued at ` 225 + Kerala VAT per copy.

A Demand Draft for an amount of 1% of the total cost of items should be enclosed with tender as EMD. Specifications and catalogue also are to be attached with tender. An agreement in the prescribed form executed and signed in Kerala Government Stamp Paper worth ` 50 should also be submitted along with the tender.

The tender without the required EMD, agreement will summarily be rejected. Late tenders will not be entertained on any account.

Any additional information required can be had from the office of the undersigned during working hours.

അടങ്കൽ തുക—` 2,25,000. നിരതദ്രവ്യം—മൊത്തം അടങ്കൽ തുകയുടെ ഒരു ശതമാനം. ദർഘാസുകൾ സമർപ്പിക്കുന്ന കവറിനുമുകളിൽ " ലബോറട്ടറി ഉപകരണങ്ങൾ വിതരണം ചെയ്യുന്നതിനുള്ള ദർഘാസ്" എന്ന് രേഖപ്പെടുത്തേ≠തും സൂപ്ര≠്, ഗവൺമെന്റ ് ഹോമിയോ മെഡിക്കൽ കോളേജ് ആശുപത്രി, കാരപ്പറമ്പ്, കോഴിക്കോട്-10 എന്ന വിലാസത്തിൽ 3-1-2012-നു പകൽ 11 മണിക്ക് മുമ്പായി ലഭിച്ചിരിക്കേ≠തുമാണ്. വൈകി ലഭിക്കുന്നവ യാതൊരു കാരണവശാലും പരിഗണിക്കുന്നതല്ല. ലഭിച്ച ദർഘാസുകൾ അന്നേ ദിവസം 11.30 മണിക്ക് ദർഘാസു കാരന്റേയോ അവർ അധികാരപ്പെടുത്തിയ വ്യ‡ികളുടെയോ സാന്നിദ്ധ്യത്തിൽ തുറന്നു പരിശോധിക്കുന്നതും ഏറ്റവും കുറഞ്ഞ നിരക്കും ഗുണമേൻമയുമുള്ള ഉൽപ്പന്നം രേഖപ്പെടു ത്തിയതുമായ ദർഘാസുകൾ അംഗീകരിക്കുന്നതുമാണ്.

ദർഘാസിന്റെ കൂടെ മൊത്തം തുകയുടെ ഒരു ശതമാനം വരുന്ന തുകയ്ക്കുള്ള ഡിമാന്റ ് ഡ്രാഫ്റ്റ് നിരതദ്രവ്യമായും കേരള സർക്കാരിന്റെ ` 50 മുദ്രപത്രത്തിൽ നി›ിത മാതൃക യിലുള്ള സമ്മതപത്രവും ഉള്ളടക്കം ചെയ്യേ≠താണ്.

ദർഘാസുകൾ ആവശ്യമുള്ളവർ സൂപ്ര≠്, ഗവൺമെന്റ ് ഹോമിയോ മെഡിക്കൽ കോളേജ് ആശുപത്രി, കാരപ്പറമ്പ്, കോഴിക്കോട്-10 എന്ന വിലാസത്തിൽ സ്വന്തം പേപ്പറിൽ അപേക്ഷിക്കേ≠താണ്. ദർഘാസ് ഫോറത്തിന്റെ വില നികുതികൾ ഒഴികെ ` 450 ആണ്. ഇത് യാതൊരു കാരണവശാലും തിരിച്ചു നൽകുന്നതല്ല. ദർഘാസുകൾ കൈമാറ്റം ചെയ്യാൻ പാടുള്ളതല്ല. ടെ≠ർ ഫോറം തപാലിൽ ലഭിക്കണമെങ്കിൽ ` 50 തപാൽ ചാർ÷ായും അയയ്ക്കേ ≠താണ്. ഡ്യൂπിക്കേറ്റ് ഫോറം ആവശ്യമുള്ളവർ ` 225 (നികുതികൾ ഒഴികെ) കൂടുതലായി അയയ്ക്കേ≠താണ്. ദർഘാസ് ഫോറം 2-1-2012-ന് പകൽ 3 മണിവരെ നേരിട്ടും 24-12-2011-ന് 3 മണി വരെ തപാൽമാർഗ്ഗവും ലഭിക്കുന്നതാണ്. തപാൽ മുഖേന ലഭിക്കണമെങ്കിൽ മണിയോർഡർ മാത്രമേ സ്വീകരിക്കുകയുള്ളു വിതരണം ചെയ്യാൻ ഉദ്ദേശിക്കുന്ന ഉപകരണത്തിന്റെ കാറ്റലോഗ്, സ്പെസിഫിക്കേഷൻ എന്നിവ ദർഘാസിന്റെ കൂടെ അടക്കം ചെയ്യേ≠താണ്.
