TENDER NOTICE

[No. 9/2011-12]

No. G1-25432/2011. 22nd November 2011.

Sealed, competitive tenders are invited from the manufacturers/suppliers for the supply instruments/ equipments for the use of the Veterinary institutions under Animal Husbandry Department, Kerala for the year 2011-12.

1. Specification of the item with rate as basic price, tax rate and total amount including any other charges and list of institutions where it has been supplied.

2. Letter from original manufacturer (in case of agents) regarding assurance about the replacement if damaged during warranty, warranty conditions, years of warranty etc.

3. Attested copy of Sales Tax Clearance and Turnover Certificate obtained from Sales Tax Authorities.

4. An affidavit in stamp paper worth ` 50 notarized by a Notary Public of either the place where the person making the affidavit is residing or of Thiruvananthapuram.

5. Copy of ISO Certification, if any:

6. Original tender form duly signed by authorized signatory.

7. Preliminary Agreement in Kerala Stamp Paper worth ` 50.

8. EMD (1% of total cost of items quoted) by means of Demand Draft in favour of the Director of Animal Husbandry, Kerala payable at Thiruvananthapuram.

9. A covering letter with name, address, Pin Code, Tel/Fax No., E-mail address and other details of the firm with the list of enclosures to be specified and addressed to the Director, Department of Animal Husbandry, Thiruvananthapuram.

Proforma for the above certificates to be attached separately.

Price of tender form:

Original—` 3,900 + VAT @ 4%.

` 3,900 + ` 156 = ` 4,056.

Duplicate—` 2,000 + VAT @ 4%.

` 2,000 + ` 80 = ` 2,080.

The cover must be effectively sealed and submitted. Submitted tender will be opened as per given above. Specifications of instruments/equipments to be supplied can be had along with the tender form, Government of Kerala Website-www.kerala.gov.in and Animal Husbandry Department website-www.ahd.kerala.gov.in. Intending tenderers may send the exact amount towards the cost of tender form. If intended to purchase on payment as money order, tender cost with the delivery charges should be send to the Director of Animal Husbandry, Vikas Bhavan, Thiruvananthapuram well in advance. Cheques, Postage Stamps, Postal Orders, Demand Drafts etc. will not be accepted as the cost of tender form nor the form be sent by Value Payable Post. Tender form can also be obtained from the Office of the Director of Animal Husbandry on payment of the cost in cash in between 11 a.m. and 4.30 p.m. Late tenders will not be considered under any circumstances. 1. The equipments should have a minimum guarantee period of two years from the date of supply.

2. Brochures & Other details should be incorporated.

3. The rates quoted should be inclusive of all taxes and other charges.

4. The rate should be quoted in Indian currency and payment will be made in Indian currency only.

5. The security deposit furnished will be released

only after satisfactory completion of the contract.

6. The rates should be for delivery F.O.R. station. Despatch rates offered by the firms are not acceptable and such tenders will be summarily rejected.

7. The quantities specified in the schedule are only approximate and they may vary according to actual need.

8. The successful tenderers should complete all supplies with in the stipulated time stated in the supply orders and payment will be effected only on supply. Advance payment is not possible. No extension of supply period will be granted.

9. The Director of Animal Husbandry reserves all rights to cancel the tender without assigning any reason thereof.

10. The Director of Animal Husbandry on his authority

has the right to inspect the manufacturing unit.

11. Payment will be made only after satisfactory supply

of the items.

12. The only jurisdiction for any dispute regarding this

purchase will be at Thiruvananthapuram.

13. The photocopies of the documents produced along with the tender should be attested by a Gazetted Officer.

14. If the date scheduled for submisssion/opening of tender is declared as holiday (local/general) the schedule will be postponed accordingly to the next working days.

15. Samples of the quoted items are to be submitted along with the tender. The quality of the sample submitted will be a criteria in finalizing the tender. The tenderer should supply the items of the same quality if awarded the supply order.

16. The items are to be supplied at the destination

within the state as in the supply order.

17. The Director of Animal Husbandry reserves the right to delete or waive any of the above special conditions.

SHORT TENDER NOTICE

Sealed and competitive tenders in the prescribed form are invited for the supply of Reagents/Chemicals/Laboratory Wares required for modernization of District Veterinary Centre, Palakkad.

Envelope containing the tender should be superscribed "Tender for the supply of Reagents/Chemicals/Laboratory Wares Tender No. 3/2011-12".

Envelope containing tender will have the following enclosures:

1. Original catalogue/brochure of the quoted item.

2. Rate of item should be quoted as basic price, rate of tax and total amount.

3. List of State/Central Govt. Organisations to which these items were supplied.

4. Original tender form duly signed by authorized signatory.

5. Preliminary Agreement in Kerala Stamp Paper worth ` 100.

6. EMD at 1% of total cost quoted item by means of Demand Draft in favour of Chief Veterinary Officer, District Veterinary Centre, Palakkad-678 001.

Tender forms are available on all working days from the undersigned. Cheques, Postal orders and demand drafts are not accepted towards the cost of tender forms. Cost of tender forms is non-refundable. Tender forms are non transferable. The tender should be accompanied by EMD of 1% by demand draft favouring the undersigned payable at Palakkad. Details of items to be supplied can be had along with the tender form and Government of Kerala website www.kerala.gov.in and Department website www.ahd.kerala.gov.in.

Price of tender form :

Original — ` 400 + 4% VAT.

Duplicate— ` 200 + 4% VAT.

Closing date and time of sale of tender forms—31-12-2011, 4 p.m.

Closing date and time of receipt of tender forms—4-1-2012, 4 p.m.

Date and time of opening of tenders—5-1-2012, 11 a.m.

Date up to which rates are to be firm—31-3-2012.

Those who require tender forms by post should sent the cost of tender forms by money order with an additional amount of ` 50 towards postage cost. Late tenders will not be accepted. The tenders will be opened in the presence of tenderers or his authorised representatives who may be present at that time.

All items must be delivered at the District Veterinary Centre, Palakkad. The items should have an expiry period of minimum two years. The supply should be completed within 4-8 weeks from the date of placing supply order.

The rates should be for destination freight prepaid in full inclusive of all taxes and duties prevailing at the ruling rate. The undersigned reserves the right to accept or reject the tenders without assigning any reason. The lowest tender shall be accepted subject to the approval of the Director of Animal Husbandry, Thiruvananthapuram.
