TERMS AND CONDITIONS

1. The tenderer (LAB) should have either NABL accreditation or should have valid traceability to the National Standards (NPL).

2. The rates quoted shall be including all the taxes and duties, expenditure towards transportation, food accommodation for team members and related expenses.

3. The equipments shall be calibrated and tested on site at Government Women & Children Hospital, Kozhikode.

4. The equipment to be calibrated and tested shall be as per Annexure A.

5. The firm shall raise invoice after calibration.

6. All the equipments shall be calibrated within one month's time from the date of work order.

7. Individual calibration report shall be provided for all the equipments.

8. Bidders shall have 3 years experience in calibration/safety test of biomedical equipments. Documentary evidence like copy of the company registration certificate and client certificate or work order should be submitted with the bid.

9. Bidders should have experience in calibrating biomedical equipments to at least one NABH accredited institutions. Documentary evidence like copy of work order should be submitted with this.

10. An agreement in Stamp Paper worth ` 100 should be produced along with tender. The Earnest Money Deposit should be produced along with the tender by way of Demand Draft.

Sealed tenders in the prescribed forms are invited for the following item to Government Medical College Hospital, Calicut-8. Entering into CAMC for O2 and N2O Pipeline systems in Major Operation Theatre, Casualty Theatre and Cardiothoracic Theatre and 22 Nos. of Boyles Anesthesia Machines.

Superscription Tender Number should be specifically noted as S3/9577/2011.
