The duly filled in application in the prescribed form (available from the Commission's website) along with the original chalan should be submitted to the Deputy Secretary (Examinations), Kerala Public Service Commission, Pattom, Thiruvananthapuram -4 on or before 27-3-2013. A copy of an answer sheet will be issued only once to a candidate. Copies of OMR answer sheets invalidated due to any16th JULY 2013] KERALA GAZETTE 956 defect will not be issued. Candidates are prohibited from applying for a copy of an answer sheet which is not their own, and legal proceedings will be initiated against those who do so.

(2) A period of forty five days time for rechecking of Answer Scripts will be allowed to candidates with effect from the date of approval of Ranked List. The candidates can avail only one chance after the approval of Ranked List.

Candidates desiring to recheck the answer script relating to the written test held for selection to the post may apply for rechecking within 45 days from the date of approval of the Ranked List.

"Applications for rechecking of answer scripts should be submitted in the prescribed form available free of cost from the Enquiry sections of the various offices of the Commission or its photocopy, or downloaded and printed in A4 size paper from the Commission's Website/www.keralapsc.gov.in photocopied there from. Applications submitted in any other manner will not be considered."

The application for rechecking along with original chalan receipt for the prescribed fee for ` 50 under the head of account "0051 - PSC 105 State PSC 99 - Examination Fee" addressed to the Deputy Secretary (Examinations), Kerala Public Service Commission, Pattom, Thiruvananthapuram -4, should reach the office on or before 27-3-2013. Application for rechecking received after the prescribed time limit and not in the prescribed form will not be entertained and fee once remitted will not be refunded.

The following is the Ranked List of candidates for selection to, the post of Typist (Direct and By Transfer Recruitment) (Category No. 334/08 & 335/08) in District Co-operative Bank in Wayanad District notified in the Gazette dated 27-9-2008 on ` 6700-18375 (PR) found suitable by the Commission and arranged in the order of merit based on the OMR Test held on 23-12-2010. (3) സാധ്യതാ പട്ടികയിൽ രജിസ്റ്റർ നമ്പർ ഉൾപ്പെട്ടിട്ടുള്ള ഉദ്യോഗാർ∞ികൾ അസ്സൽ പ്രമാണ പരിശോധനയ്ക്ക് ഹാജരാകുമ്പോൾ 6 മാസത്തിനകം എടുത്ത ഒരു ഫോട്ടോയും (പാസ്പോർട്ട് സൈസ്) തിരിച്ചറിയൽ രേഖയുടെ അസ്സലും ശരിപ്പകർപ്പും ഹാജരാ ക്കണം. ഇലക്ഷൻ എെഡന്റിറ്റി കാർഡ്, പാൻ കാർഡ്, ഡ്രൈവിംഗ് ലൈസൻസ്, സാമൂഹ്യ ക്ഷേമ വകുപ്പ് വികലാംഗർക്കായി നൽകുന്ന ഫോട്ടോ പതിച്ച തിരിച്ചറിയൽ കാർഡ്, ദേശ സാൽകൃത ബാങ്ക് നൽകുന്ന ഫോട്ടോ പതിച്ച പാസ്സ് ബുക്ക്, സർക്കാർ ജീവനക്കാർക്ക് അവർ ജോലി ചെയ്യുന്ന സ്ഥാപനം നൽകുന്ന ഫോട്ടോ പതിച്ച തിരിച്ചറിയൽ കാർഡ്, വിമു‡ഭട∑ാരുടെ ഡിസ്ചാർ÷് സർട്ടിഫി ക്കറ്റ്/ജില്ലാ സൈനിക ക്ഷേമ ആഫീസർ നൽകുന്ന ഫോട്ടോ പതിച്ച എെഡന്റ ിറ്റി കാർഡ്, മോട്ടോർ വാഹന വകുപ്പ് നൽകുന്ന ക≠ക്ടർ ലൈസൻസ്, Scheduled Banks/ Kerala State Co-operative Bank/District Co- operaive Banks നൽകുന്ന ഫോട്ടോ പതിച്ച പാസ്സ് ബുക്ക്, പൊതുമേഖലാ സ്ഥാപനങ്ങൾ, (വിവിധ കമ്പനികൾ/കോർപ്പറേഷനുകൾ/ ബോർഡുകൾ/ അതോറിറ്റികൾ) സർക്കാർ സ്വയംഭരണ സ്ഥാപനങ്ങൾ എന്നിവ അവ രുടെ ജീവനക്കാർക്ക് നൽകുന്ന ഫോട്ടോ പതിച്ച എെഡന്റ ിറ്റി കാർഡ്, കേരള സംസ്ഥാനത്തിലെ സർവ്വകലാശാല ജീവന ക്കാർക്ക് നൽകുന്ന ഫോട്ടോ പതിച്ച എെഡന്റിറ്റി കാർഡ്, വികലാംഗരായ ഉദ്യോ ഗാർ∞ികൾക്ക് മെഡിക്കൽ ബോർഡ് നൽകുന്ന ഫോട്ടോ പതിച്ച മെഡിക്കൽ സർട്ടിഫിക്കറ്റ്, അഡ്വക്കേറ്റായി Enroll ചെയ്തവർക്ക് Bar Council നൽകുന്ന ഫോട്ടോ പതിച്ച എെഡന്റ ിറ്റി കാർഡ്, കേന്ദ്ര ഗവൺമെന്റ ് നൽകുന്ന ആധാർ കാർഡ് (Aadhaar card) എന്നിവയിലേതെങ്കിലും തിരിച്ചറിയൽ രേഖയായി ഹാജരാക്കാവു ന്നതാണ്.

(4) ഉത്തരക്കടലാസ് പുനxപരിശോധനയ്ക്കുള്ള അപേക്ഷകളും ഒ. എം. ആർ. ഉത്തരക്കടലാ സിന്റെ പകർപ്പ് ലഭിക്കുന്നതിനുള്ള അപേക്ഷ കളും റാങ്ക് ലിസ്റ്റ് പ്രസിദ്ധീകരിച്ചതിനുശേഷം മാത്രമേ പരിഗണിക്കുകയുള്ളു.

(5) ഉത്തരക്കടലാസ് അസാധുവാക്കപ്പെട്ടവരുടെ രജിസ്റ്റർ നമ്പർ അടങ്ങുന്ന വി⁄ാപനം ഇൻവാലിഡേറ്റഡ് നോട്ടിഫിക്കേഷൻ ഇതോ ടൊപ്പം പ്രസിദ്ധീകരിച്ചിട്ടു≠്. [ER VII (3) 9448/12/EW dated 29-12-2012].

(6) സാധ്യതാ പട്ടികയിൽ ഉൾപ്പെട്ടിട്ടുള്ള ഉദ്യോഗാർ∞ികൾക്ക് പ്രമാണങ്ങളുടെ പരിശോധനയ്ക്കും അർഹതാ നിർണ്ണയ ത്തിനും ഹാജരാകാൻ വ്യ‡ിഗത മെമ്മോ അയയ്ക്കുന്നതാണ്.

1. പാസ് പോർട്ട് സൈസ് ഫോട്ടോ—1 എണ്ണം ആറു മാസത്തി ന ക ം എ ട ുത്ത പാസ്പോർട്ട് സൈസ് ഫോട്ടോ (ഉദ്യോഗാർ∞ികളുടെ പേരും ഫോട്ടോ എടുത്ത തീയതിയും ഫോട്ടോയിൽ പ്രിന്റ ് ചെയ്തിരിക്കണം). സ്റ്റാമ്പ് സൈസ് ഫോട്ടോ ഹാജരാക്കാൻ പാടില്ല. ഫോട്ടോ ഇളം നിറത്തിലുള്ള പ› ാ ത്ത ല ത്തേ ാ ട ു ക ൂ ട ി യ ത ു ം മുഖം പൂർണ്ണമായും ഫോട്ടോയുടെ മദ്ധ്യഭാഗത്ത് പതിഞ്ഞിരിക്കേ≠തും േ ത ാ ൾ ഭാഗവും കണ്ണുകളും വ്യ‡മായി കാണത്തക്കവിധത്തി ലുള്ളതും അളവിലുള്ളതുമായിരിക്കണം. മേൽ നിബന്ധനകൾ പാലിക്കാതെ ഉള്ള ഫോട്ടോ ഹാജരാക്കുന്ന ഉദ്യോഗാർ∞ി കളെ പ്രായോഗിക പരീക്ഷയിൽ പങ്കടുപ്പിക്കുന്നതല്ല.

2. തിരിച്ചറിയൽ രേഖ— കുറിപ്പ് 3-ൽ പ്രസ്താവിച്ച പ്രകാരം.

3. ജനന തീയതി, വയസ്, വിദ്യാഭ്യാസ േ യ ാ ഗ ്യ ത — ബന്ധപ്പെട്ട സ്കൂൾ രേഖകളുടെ അ   ല ു ം സ ്വ യ ം സാക്ഷ്യപ്പെടുത്തിയ പകർപ്പും (എസ്. എസ്. എൽ. സി.) 4. പി.എച്ച്. സർട്ടിഫിക്കറ്റ് — വൈകല്യം തെളിയിക്കുന്ന സർട്ടിഫിക്കറ്റ്.

5. സർവ്വീസിലുള്ള ഉദ്യോഗാർ∞ികൾ—ബന്ധപ്പെട്ട വകുപ്പ് മോധാവിയിൽ നിന്നോ ഓഫീസ് മേധാവിയിൽ നിന്നോ ലഭിച്ച രസീത ് (ഒറിജിനൽ).

മുകളിൽ പറയും പ്രകാരമുള്ള സർട്ടിഫിക്കറ്റുകളും രേഖകളും വെരിഫിക്കേഷൻ സമയത്തിനുശേഷം ഹാജരാക്കാൻ അവസരം നൽകുന്നതല്ല.

The following is the Short List containing the Register Numbers of candidates who are found provisionally eligible to be called for interview for selection to the post of Laboratory Technician Grade II (` 6080-9830) Laboratory Assistant Grade II (` 5650-8790) (NCA-Ezhava) in Animal Husbandry Department (Category No. 44/2010) in Kasaragod District, on the basis of the OMR Test held on 15-12-2011.

The Register Numbers are arranged in their numerical sequence and the arrangement does not in any way indicate their respective rank.

The letter 'B' should be prefixed to all Register Numbers to denote Kasaragod District.

Note:—1. Inclusion of Register Numbers in this list is purely provisional subject to scrutiny and admissions of applications on absolute basis. Inclusion of Register Numbers in the short list does not confer any right to the candidate to be called for interview or for subsequent inclusion in the Ranked List.

2. The Short List is prepared based on the details furnished by the candidate in the applications and subsequent claims made will not be entertained at any stage.

3. The candidates whose Register Numbers are included in the short list are directed to send self attested copies of documents to prove their claims made in the applications, i.e., qualification, date of birth, community and other benefits, if any, to which they are entitled to, the District Officer, Kerala Public Service Commission District Office,Tiger Hills, Municipal Office Road, Pulikunnu, Kasaragod within 15 days from the date of publication of this notification.

5. According to the existing procedure, revaluation of answer scripts is not allowed. But answer scripts will be rechecked and photocopy of answer script will be issued after the publication of the Ranked List, if applied as per rules. Detailed instructions in this regard will be issued in the notification of the Ranked List. Applications for re-checking/ photocopy of invalidated answer script will not be considered.
