12 KERALA GAZETTE [PART III ഇതിലേക്കായുള്ള ദർഘാസുകൾ നിർദ്ദിഷ്ട ഫോറത്തിൽ പ്രത്യേകം മുദ്രവച്ച കവറുകളിൽ സമർപ്പിക്കേ≠I. The tenderer should obtain the HT approval of the Electrical Inspectorate and payment made only after submitting completion certificate.
II. Detailed tender schedule & tender form will be issued from District Hospital, Kottayam office in the working hours.
III. The Superintendent, District Hospital, Kottayam is the authority to accept and rejection of tender with his decision.
IV. All the tenderers should bring the details of their previous work experience in Kottayam DHK jurisdiction.
V. After opening the tender the tender evaluation committee will negotiate all the parties and lowest and reasonable (with provision work exp.) may accept due to the conformation of Electrical Inspectorate.
VI. The tenderer shall pay the VAT as per rules and 1 % of TDS amount to income tax and 1 % to welfare fund.
VII. Tender have a license of PWD A grade electrical contractor, and approval of Electrical Inspectorate, Kottayam.

തൃശ്ശൂർ ജില്ലയിലെ ആരോഗ്യ വകുപ്പിലെ ഉപയോഗ ശൂന്യമായ വാഹനങ്ങളുടെ ലേല നടപടികൾക്കുവേ≠ി ഉപഭോ‡ാക്കളിൽ നിന്നും മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചു കൊള്ളുന്നു.
താണ്. ദർഘാസ് ഫോറങ്ങൾ ഈ ആഫീസിലെ ചാർ÷് വഹി ക്കുന്ന സെക്ഷനിൽനിന്നും 20-5-2014 മുതൽ 29-5-2014 വരെ പ്രവർത്തി ദിവസങ്ങളിൽ ലഭിക്കുന്നതാണ്. ദർഘാസ് ഫോറങ്ങൾക്ക് നിയമപ്രകാരം നിശ്ചയിച്ചിരിക്കുന്ന വിലയും വില്പന നികുതി പുറമേയും ഒടുക്കിയാൽ ലഭിക്കുന്നതാണ്.
മുദ്രവച്ച ദർഘാസുകൾ 2014 ജൂൺ മാസം 21-ന് ഉച്ചയ്ക്ക് 12 മണിവരെ സ്വീകരിക്കുന്നതും അന്നേ ദിവസം ഉച്ചയ്ക്ക് 2 മണിക്ക് ബന്ധപ്പെട്ട ഹാജരുള്ള കക്ഷി കളുടെ സാന്നിദ്ധ്യത്തിൽ തുറന്ന് പരിശോധിക്കുന്നതുമാണ്.
ഓരോ ദർഘാസിനൊപ്പവും 1-4-2014-നോ അതിന് ശേഷമോ ലഭിച്ചിട്ടുള്ള സോൾവൻസി സർട്ടിഫിക്കറ്റും ബന്ധപ്പെട്ട രജിസ്ട്രാറിൽ നിന്നും ലഭിച്ചിട്ടുള്ള കടബാദ്ധ്യത രഹിത സർട്ടിഫിക്കറ്റും ഹാജരാക്കേ≠താണ്.
സോൾവൻസി സർട്ടിഫിക്കറ്റിന്റെ അസ്സലും സാക്ഷ്യ പ്പെടുത്തിയ പകർപ്പും മതിയായ നിരതദ്രവ്യ തുകയും മുദ്രപത്രത്തിൽ നി›ിത മാതൃകയിൽ കരാർ, ക≠ീഷൻസ് ടു ടെ≠ർ, ദർഘാസ് ഫോറത്തിന്റെ പകർപ്പ് എന്നിവയും ദർഘാസിനൊപ്പം സമർപ്പിക്കേ≠താണ്. സമർപ്പിച്ചിട്ടുള്ള ദർഘാസുകൾ കാരണങ്ങൾ ഒന്നും കാണിക്കാതെ തന്നെ നിരാകരിക്കുന്നതിനും/സ്വീകരിക്കുന്നതിനുമുള്ള പരിപൂർണ്ണ അധികാരം ഈ ആഫീസിലെ മേലധികാരിയിൽ നിക്ഷിപ്ത മാണ്. ദർഘാസ് സ്വീകരിക്കപ്പെടുന്ന വ്യ‡ി/സ്ഥാപനം മൊത്തം ദർഘാസ് തുകയും 5%-ൽ കുറയാത്ത സംഖ്യ സെക്യൂരിറ്റി ആയും നി›ിത മാതൃകയിൽ എഗ്രിമെന്റും സമർപ്പിക്കേ≠താണ്. കൂടുതൽ വിവരങ്ങൾ ഈ ആഫീസിലെ മേലധികാരിയിൽ നിന്നും നേരിട്ട് അറിയാവുന്നതാണ്.
