തിരുവനന്തപുരം ജില്ലാ മാനസികാരോഗ്യ പരിപാടിയുടെ മാനസികാരോഗ്യ ക്ലിനിക്കുകൾ, ക്യാമ്പുകൾ, ബോധവൽക്കരണ ക്ലാസുകൾ, പരിശീലന പരിപാടി എന്നിവ നടത്തുന്നതിന് വേ≠ി കരാർ അടിസ്ഥാനത്തിൽ വ≠ി വാടകയ്ക്ക് എടുക്കുന്നതിലേക്കായി സ്വകാര്യ വാഹന ഉടമകളിൽ നിന്ന് മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു.
വാഹനത്തിന്റെ വിശദവിവരം
1. ഏഴ് പേർക്ക് ഇരിക്കാവുന്നതും എയർക≠ീഷൻഡു മായിരിക്കണം.
2. 2012-ലോ ശേഷമോ നിർമ്മിച്ചത്.
3. മഹീന്ദ്ര-സൈലോ/സ്കോർപ്പിയോ/ടോയോട്ടോ-ഇന്നോവ/ ടവേര.
അടങ്കൽ തുക—` 5,10,600.
ദർഘാസുകളിൽ ടെ≠ർ നമ്പർ രേഖപ്പെടുത്തി 'നോഡൽ ഓഫീസർ, ജില്ലാ മാനസികാരോഗ്യ പരിപാടി, തിരുവനന്തപുരം' എന്ന വിലാസത്തിൽ 11-3-2015 വൈകുന്നേരം 3 മണിക്ക് മുമ്പായി സമർപ്പിക്കേ≠താണ്. സ്വീകാര്യമായ ദർഘാസുകൾ നൽകിയവരുടെയോ അവർ അധികാര പ്പെടുത്തിയവരുടെയോ സാന്നിദ്ധ്യത്തിൽ 12-3-2015 വൈകുന്നേരം 3 മണിക്ക് മുദ്രവച്ച ദർഘാസുകൾ തുറക്കുന്നതാണ്.
ദർഘാസുകൾ വിൽക്കുന്ന അവസാന തീയതി 10-3-2015 വൈകുന്നേരം 3 മണിവരെയാണ്. ദർഘാസ് ഫോറം ആവശ്യമുള്ളവർ അതിന്റെ വില 'ജില്ലാ മാനസികാരോഗ്യ പരിപാടി, മാനസികാരോഗ്യകേന്ദ്രം, തിരുവനന്തപുരം' ഓഫീസിൽ നിന്ന് പ്രവൃത്തി ദിവസങ്ങളിൽ നേരിട്ട് അടച്ച് വാങ്ങാവുന്നതാണ്. (രാവിലെ 9 മണി മുതൽ വൈകുന്നേരം 3 മണി വരെ).
ടെ≠റിനെ സംബന്ധിച്ചുള്ള പ്രത്യേക നിബന്ധനകളും വിശദാംശങ്ങളും ഫോറത്തിനോടൊപ്പം ലഭിക്കുന്നതാണ്.
10 KERALA GAZETTE [PART III No. 32/2015/DMHP/TVPM. 10th February 2015.Sealed tenders are invited from Vehicle owners (private) for the hiring of vehicle on contract for District Mental Health Programme, Thiruvananthapuram for conducting mental health clinics, awareness classes, camps and training programmes at different places in Thiruvananthapuram District.
SPECIFICATIONS
1. 7 seater with A/c.
2. Year of manufacture 2012 onwards.
3. Tavera/Mahendra Xylo or Scorpio/Toyoto Innova P. A. C.—` 5,10,600.
EMD—1%.
Cost of tender form:
Original—` 1,100 + 13. 5% VAT.
Duplicate—` 600 + 13. 5% VAT.
Last date and time of sale of tender form—10-3-2015 at 3 p. m.
Last date and time of receipt of tender—11-3-2015 at 3 p.m.
Date and time of opening of tender—12-3-2015 at 3 p. m.
Tender forms will be obtained from the District Mental Health Programme (DMHP) on payment of cash during office hours (9 a. m. to 3 p. m.).
Tender No. and date should be superscribed on such envelope and addressed to the Nodal Officer, District Mental Health Programme, Thiruvananthapuram. Late tenders will not be accepted. Cost of tender form will not be returned under any circumstances. Tender should be accompanied by Earnest Money Deposit in the form of Demand Draft drawn in favour of "Nodal Officer, District Mental Health Programme, Thiruvananthapuram", which should not be less than one per cent of the price quoted. The undersigned has the right to reject any offer without assigning reason thereof.
Special terms and conditions of the tender shall be obtained with the tender form on working days.
Postal Orders, Postal Stamps, Bank Draft, Banker's cheques will not accepted towards the cost of tenders.
