No. 376556 of March 1991 , also known as Sathiabhama in the Election Identity Card No. KL/ 01 / 008 /405346 dated 23-1-1999 issued by the Electoral Registration Officer for Taliparamba Constituency as Jessy Reji in the Aadhaar No. 7397 9498 9219 issued from Unique Identification Authority of India and as Jessy in the Marriage Certificate Reference No. 388 dated 30-12-2014 issued by the Parish Priest , Diocese of Kannur is one and the same person . I have embraced Christian Religion from Hindu - Thiyya Community as per Baptism Certificate No. on Register 3183 dated 30-12-2014 issued by the Pariest Diocese of Kannur with the name Jessy Maria. Hereafter I will be a member of Christian Religion and will be known by the name Jessy Reji only on consequent on my marriage and will sign accordingly.
This change will come into effect in all records related to me .

No. 6 , dated 24-12-2008 , issued by Taluk Supply Officer, Thalassery is one and the same person . Hereafter I will be known by the name Fathmathul Mazni Muzammil only.
This change will come into effect in all records related to me .
