C. S.C. Community as per Discharge Certificate No. 5547 dated 19-12-2014 issued by the President/ Secretary , Therbiyathul Islam Sabha , Mukhadar, Calicut with a new name Hiba Fathima. Hereafter I will be a member of Islam Religion and have changed my name as Hiba Fathima Mahroof on consequent to my marriage and will sign accordingly .
This change will come into effect in all records related to me .
