കോഴിക്കോട് ജില്ലാ ടി. ബി. സെന്ററിലേക്ക് താഴെ പറയുന്ന ലബോറട്ടറി സാധനങ്ങൾ വിതരണം ചെയ്യുന്നതിന് നി›ിത ഫോറത്തിൽ മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചു കൊള്ളുന്നു.

ദർഘാസ് സമർപ്പിക്കുന്ന കവറിനുമുകളിൽ സി2/68/2015 എന്ന് വ്യ‡മായി കാണിച്ചിരിക്കേ≠താണ്. ഓരോ ദർഘാസുകളും കോഴിക്കോട് ജില്ലാ ടി. ബി. ഓഫീസറുടെ മേൽവിലാസത്തിൽ പ്രത്യേകം കവറിലാക്കി സീൽ ചെയ്ത് 10-6-2015-ന് ഉച്ചയ്ക്കുശേഷം 5 മണിവരെ സമർപ്പിക്കാവുന്നതാണ്. 11-6-2015-ന് പകൽ 11 മണിക്ക് ദർഘാസുകൾ നിയമാനുസരണം തുറക്കുന്നതായിരിക്കും. വൈകി ലഭിക്കുന്ന ദർഘാസുകൾ സ്വീകരിക്കുന്നതല്ല.

തല്ല. ദർഘാസ് ഫോറങ്ങൾ കൈമാറ്റം ചെയ്യാൻ പാടില്ലാത്തതാ കുന്നു. ഓരോ സാധനത്തിന്റെയും സ്പെസിഫിക്കേഷൻ ദർഘാസിൽ രേഖപ്പെടുത്തേ≠താണ്. ആവശ്യപ്പെടുന്നപക്ഷം സ ാ ധ ന ങ്ങളുടെ സാമ്പിൾസ് ദർഘാസുകാരൻ സ്വന്തം ചെലവിൽ പരിശോധനയ്ക്കായി ഹാജരാക്കേ≠തും തിരിച്ചു കൊ≠ു പോകേ≠തുമാണ്. സാധനങ്ങളുടെ വിതരണം ഉത്തരവ് ലഭിക്കുന്ന മുറയ്ക്ക് ആവശ്യാനുസരണം നിർവ്വഹിക്കേ≠താണ്.

പൂരിപ്പിച്ച ദർഘാസ് ഫോറത്തിനോടൊപ്പം സാധനങ്ങളുടെ മൊത്തം വിലയുടെ 2.5% നിരതദ്രവ്യത്തിന് കോഴിക്കോട് ജില്ലാ ടി. ബി. ഓഫീസറുടെ പേരിൽ എടുത്ത ഡ്രാഫ്റ്റും ` 500 വിലയുള്ള കേരള സംസ്ഥാന മുദ്രപ്പത്രത്തിൽ രേഖപ്പെടുത്തിയ ഒരു എഗ്രിമെന്റും ഹാജരാക്കേ≠താണ്.

ദർഘാസിൽ പറയുന്ന വില നിരക്കുകൾ 30-6-2016 വരെ മാറ്റം വരുത്തുവാൻ പാടില്ലാത്തതാകുന്നു. യാതൊരു കാരണവശാലും ഏതൊരു ടെ≠റും കാരണം കാണിക്കാതെ നിരസിക്കാനോമാറ്റിവയ്ക്കാനോ ഉള്ള അധികാരം കോഴിക്കോട് ജില്ലാ ടി. ബി. അടങ്കൽ തുക—` 9 ലക്ഷം. ഓഫീസറിൽ നിക്ഷിപ്തമായിരിക്കും. എല്ലാ ദർഘാസുകൾക്കും ബാധകമായ സർക്കാർ നിയമങ്ങൾ ഈ ദർഘാസിനും ബാധകമാണ്. കൂടുതൽ വിവരങ്ങൾ ജില്ലാ ടി. ബി. ദർഘാസുകൾ ആവശ്യമുള്ളവർ അതിന്റെ വിലയായ ` 1,800 (ഡ്യൂπിക്കേറ്റ് ഒന്നിന് ` 900) അടച്ച് 4-6-2015-ന് പകൽ 3 മണിക്ക് മുമ്പായി കോഴിക്കോട് ജില്ലാ ടി. ബി. ഓഫീസിൽ ഓഫീസിൽനിന്ന് പ്രവൃത്തി സമയങ്ങളിൽ ലഭിക്കുന്നതാണ്. ഇന വിവരം LIST OF ITEMS

of the following items to District TB Centre, Kozhikode.

The tenders should be superscribed C2/68/2015 and addressed to the District TB Officer, Kozhikode. The last date of the receipt of the tenders will be at 5 p.m. on 10-6-2015. Late tenders will not be accepted. The tenders will be opened at 11 a.m. on 11-6-2015 in the presence of such of the tenderers or their authorized representatives who may be present at that time.

Esimated cost—` 9 Lakhs.

Intending tenderers may on application to the the District TB Officer, District TB Centre, Kozhikode obtain the requisite tender form in which tenders should be submitted. Application forms should be accompanied by a cash remittance of ` 1,800 which is the price fixed for a form/set of forms and which is not refundable under any circumstances. The tender forms are not transferable. Sale of tender forms will be closed at 3 p.m. on 4-6-2015. Cheques, Postage Stamps etc. will not be accepted towards the cost of tender forms nor will the forms be sent per V. P. P. Duplicate tender forms if required will be issued at ` 900 per copy. Specification of each item should also be mentioned in the tender. Samples should be produced if called for and unapproved samples got back by the tenderers at their own cost. Supply of the items should be made according to the requirement on request.

Intending tenderers should deposit an amount of 2.5% of the total cost quoted rate as Earnest Money Deposit by Demand Draft in favour of the District TB Officer, District TB Centre, Kozhikode together with an agreement in Kerala Stamp Paper worth ` 500. The rates quoted shall remain firm for acceptance till 30-6-2016. The District TB Officer, District TB Centre, Kozhikode will have power to postpone/cancel the tender. The Government rules applicable to all tenders will be applicable to this tender also.
