TENDER NOTICE
[No. 08/2015-16]
No. G1.13520/2015/Stores. 1st July 2015.
Sealed and competitive tenders are invited from the manufactures/authorized dealers or suppliers under "Two cover system" for the supply of 2ml. disposable nylon syringe with 21 guage needles for the use of various Veterinary Institutions of the Department for the year 2015-16.
6. All documents furnished should be attested by a Notary.
7. An affidavit in Stamp paper worth ` 100 notarised by a Notary, where the person making affidavit is residing or of Thiruvananthapuram signed by a responsible person (Managing Director or Authorised Supplier/Dealer) specifying his status, age, residential address.
First Cover (Cover A-Technical) will have the following
enclosures:
1. Detailed specification of item, without rate and list of Institutions where supply has been effected. Make of the item quoted should be mentioned.
8. A declaration of the supplier/manufacturer making the supply of the materials to the effect that the supply of the materials will be done within 30 days from the date of receipt of the supply order.
9. Samples should be provided with cover A.
Second Cover (Cover B-Financial) will have the following
enclosures:
2. Original/attested copy of manufacturing license in case of manufacturer. In case of supplier, original or attested copy of letter from the original manufacturer authorizing the supplier to quote on their behalf.
3. Original/attested copy of turnover certificate from Chartered Accountant.
4. Copy of the Sales Tax clearance certificate.
5. Copy of ISO /GMP/ WHO certificate if any of the manufacturer.
1. Rate should be quoted as basic price, rate of tax and total amount.
2. Original tender form duly signed by authorized signatory.
3. Preliminary agreement in Kerala Stamp paper worth ` 100.
4. EMD as Demand Draft drawn in favour of the Director, Animal Husbandry Department, Kerala. (EMD—` 2,500).
The items should be supplied at the three destinations:—District Veterinary Store, Thiruvananthapuram, Thrissur and Kozhikode as per the instructions in the Supply Order issued.
Sample of 5 (five) numbers should be submitted along with the tender:
Special Conditions
Both the covers (A&B) must be effectively sealed and put in another cover, which should also be sealed and submitted. Cover A&B will be opened as per the schedule given below. Cover B of those tenderers, those who satisfy the conditions in Cover A and passing the quality of the samples produced, will only be opened. The unopened B Cover of the rejected tenderers will be returned on written request only after completing the purchase procedures. Details of 2ml. disposable nylon syringe with 21 gauge needle to be supplied can be had from the tender form and Govt. of Kerala website www.kerala.gov.in. Intending tenderers may send the exact amount towards the cost of tender form as Money Order with Postal Charges for sending the tenders well in advance. Cheques, Postage stamps, Postal Orders, Demand Drafts, etc. will not be accepted as the cost of tender form nor the form be sent by Value Payable Post. Tender form can also be obtained from the office of the Director of Animal Husbandry on payment of the cost in cash from 10.30 a.m. to 3 p.m. on working days. Late tenders will not be considered under any circumstances. Tenders should be superscribed as "Tender No. 08/2015-16 for the supply of 2ml. Nylon disposable Syringe with 21 Guage needles to the Animal Husbandry Department, Kerala for the year 2015-16".
Price of tender form :
Original—` 500 + 5% VAT = ` 525.
Duplicate—` 300 + 5% VAT= ` 315.
Last date and time for sale of tender forms—
17-8-2015 at 3 p. m.
Due date and time for receipt of tenders (Cover A&B)—18-8-2015 at 4 p. m.
Date and time for opening Cover A—
19-8-2015 at 11 a. m.
Date of publishing list of tenders eligible for opening Cover B—19-8-2015 at 3 p. m.
Date and time for opening Cover B—22-8-2015 at 11 a. m.
Date upto which the rates are to be firm for acceptance—31-3-2016.
1. The rate quoted should be as basic price, rate of tax & total amount.
2. Offers should be from ready stock in the original packing of the manufacturer.
3. Preference will be given to tenderers with ISO certificates.
4. Director of Animal Husbandry has the right to finalise the tender on the basis of the quality of the sample produced.
5. The Security Deposit furnished will be released only after satisfactory completion of the contract.
6. The rates should be for delivery f.o.r. at the destinations.
7. Ex-go down rates and f.o.r. stations of dispatch rates offered by the firms are not acceptable and such tenders will be summarily rejected.
8. On the label of packing, it should be inscribed "for the use of Animal Husbandry Department, Kerala''.
9. The quantities specified in the schedule are only approximate and they may vary according to actual need and budget provision.
10. The successful tenderers should complete all supplies within the stipulated time stated in the supply orders. No extension of supply period will be granted.
11. The non-availability of raw materials, price hike of raw materials, transportation problems, etc. will not be accepted as reasons for non- supply or late supply.
12. Successful tenderers should supply the items at the approved rates and conditions to the local self Governments and other Departmental institutions as per their request.
13. The rates should be quoted in Indian Currency, and payment will be made in Indian currency.
14. The Director of Animal Husbandry reserves all rights to cancel the tender without assigning the reason thereof.
15. The Director of Animal Husbandry on his authority has the right to inspect the manufacturing unit.

17. The photocopies of the documents produced along with the tender would be attested by a Gazetted Officer.
18. If any of the dates scheduled for receipt and opening of tenders declared holding (local/ general) the receipt and opening will be on the next working day.
19. The items should supply at the said destinations of the department on time.
20. The Director of Animal Husbandry reserves the right to delete or waive any of the above Special Conditions.

Sealed competitive tenders are invited for the purchase of below mentioned Packing Material for Mastitis Kit for the use of the Institute of Animal Health & Veterinary Biologicals, Palode, Thiruvananthapuram.
Tender No.—VI/2015-16
The tender should be superscribed "Tender No. VI/2015-16 Supply of Packing Material for Mastestis Kit" and should be addressed to the Director, Institute of Animal Health and Veterinary Biologicals, Palode, Pacha P. O., Thiruvananthapuram, Kerala, Pin-695 562.
Last date for the receipt of the tender is 21-8-2015 at 11 a. m. Late tenders will not be accepted. The tenders will be opened at 11.30 a.m. on 21-8-2015 in the presence of the tenderers or their authorised representatives may present at that time of opening of tender.
Intending tenderers may apply to the Director, Institute of Animal Health and Veterinary Biologicals, Palode, Pacha P. O., Thiruvananthapuram, Pin-695 562 for obtaining the prescribed tender form on which the tender should be submitted. Application for the tender form should be accompanied by a cash remittance of ` 400 and VAT 5% which is the price fixed for a set of tender form and which is not refundable. The tender forms are not transferable. Sale of the tender forms will be closed at 3 p. m. on 20-8-2015. Those who have wish to get the tender form by post may sent ` 500 by M.O. which includes cost, taxes, postal charges etc. Cheques, Postal Stamps, Demand Drafts etc. will not be accepted towards the cost of tender forms. The form will not sent by V. P. P. Duplicate forms if required will be issued at ` 200 and VAT 5% extra per copy.
