LA, Kasargode, Election Identity Card No. KL/0l/001/132256 dated 15-2-1999 issued by the Electoral Registration Officer, for Manjeswar Constituency, Ration Card No. 2468006322, dated 17-10-2011 (Sl. No. 1) issued by the Taluk Supply Officer, Kasaragode and as Prashantha Shetty in the Passport No. J 3823873 issued on 11-10-2010 by the Assistant Passport Office, Kozhikode is one and the same person. Hereafter I will be known by the name Prashantha Shetty only and will sign accordingly.
