കണ്ണൂർ ഗവൺമെന്റ ് ആയുർവേദ കോളേജ് ആശുപത്രിയിലേക്ക് രോഗികൾക്ക് വിതരണം ചെയ്യുന്നതിനായി 2016-17 സാമ്പത്തിക വർഷത്തേക്ക് നല്ലയിനം റൊട്ടി വിതരണം ചെയ്യുവാൻ താൽപര്യമുള്ളവരിൽനിന്നും മുദ്രവച്ച ദർഘാസുകൾ ക്ഷണിച്ചുകൊള്ളുന്നു. ദർഘാസ് കവറിനുപുറത്ത് "നല്ലയിനം റൊട്ടി വിതരണം ചെയ്യുന്നതിനുള്ള ദർഘാസ്" എന്ന് രേഖപ്പെടുത്തേ≠താണ്. ഒരു വർഷത്തേക്ക് വിതരണം ചെയ്യേ≠ റൊട്ടിയുടെ അളവ് ഏകദേശം 5475 കിലോഗ്രാം ആണ്. ഒരുകിലോ റൊട്ടി വിതരണം ചെയ്യുന്നതിനുള്ള നിരക്ക് ദർഘാസിൽ പ്രത്യേകം രേഖപ്പെടുത്തേ≠താണ്.
ഓഫീസിൽ ലഭിച്ചിരിക്കണം. വൈകിക്കിട്ടുന്ന ദർഘാസുകൾ സ്വീകരിക്കുന്നതല്ല. ദർഘാസ് വാങ്ങിയവരുടെയോ അവരുടെ പ്രതിനിധികളുടെയോ സാന്നിദ്ധ്യത്തിൽ 26-5-2016-ന് 3.30 മണിക്ക് ദർഘാസുകൾ തുറക്കുന്നതായിരിക്കും.
അടങ്കൽ തുക—` 3,00,000.
ദർഘാസ് ഫോറത്തിന്റെ വില ` 600 + 5% മൂല്യവർദ്ധിത നികുതി, ആകെ ` 630. ഫോറം തപാലിൽ ലഭിക്കുവാൻ വിലയോടൊപ്പം ` 50 അധികമായി മണിയോർഡർ അയയ്ക്കേ≠താണ്. ദർഘാസ് ഫോറത്തിന്റെ വില നി›ിതവും ഒരിക്കലും തിരിച്ചുനൽകാത്തതുമാണ്.
ദർഘാസ് മറ്റൊരാളുടെ പേരിലേക്ക് മാറ്റാവുന്നതല്ല.
ദർഘാസ് ഫോറങ്ങളുടെ വില്പന ഈ ഓഫീസിൽനിന്നും 5-5-2016-ന് രാവിലെ 10 മണിമുതൽ ആരംഭിക്കുന്നതും 25-5-2016-ന് വൈകുന്നേരം 3 മണിക്ക് അവസാനിക്കുന്ന തുമാണ്. ചെക്ക്, പോസ്റ്റൽ ഓർഡർ, ഡിമാന്റ ് ഡ്രാഫ്റ്റ് തുടങ്ങിയവ ദർഘാസ് വിലയായി സ്വീകരിക്കുന്നതല്ല.
ഫോറങ്ങൾ വി. പി. പി. ആയി അയയ്ക്കുന്നതല്ല. ഡ്യൂπിക്കേറ്റ് ഫോറങ്ങൾ ആവശ്യമെങ്കിൽ ` 300 + 5% മൂല്യവർദ്ധിത 26th APRIL 2016] STORES PURCHASE 506 നികുതി. ആകെ ` 315-ന് ലഭിക്കുന്നതാണ്. പൂരിപ്പിച്ച ദർഘാസിനോടൊപ്പം രേഖപ്പെടുത്തിയ മൊത്തം തുകയുടെ 2.5% തുകയുടെ നിരതദ്രവ്യമായും ` 200 വിലയുള്ള കേരള മുദ്രക്കടലാസിൽ അപ്പന്റിക്സ്-17 പ്രകാരമുള്ള പ്രിലിമിനറി എഗ്രിമെന്റും സമർപ്പിക്കേ≠താണ്.
Kerala Stamp Paper worth ` 200 together with an EMD of 2.5% of the total amount quoted should be attached with the tender form by way of Demand Draft/Postal Order.
Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram is the competent authority to accept or reject or postponement of tender. Further details can be had from the Hospital Office during the office time.
26-5-2016-ന് സർക്കാർ അവധി പ്രഖ്യാപിക്കുകയാണെങ്കിൽ
അടുത്ത പ്രവൃത്തിദിവസം 3.30-ന് ടെ≠ർ തുറക്കുന്ന തായിരിക്കും. ടെ≠ർ അംഗീകരിക്കാനോ നിരസിക്കാനോ ഉള്ള പൂർണ്ണ അധികാരം സൂപ്ര≠ിനായിരിക്കും. കൂടുതൽ വിവരങ്ങൾ ആശുപത്രി ഓഫീസിൽ നിന്നും പ്രവൃത്തിദിവസങ്ങളിൽ അറിയാവുന്നതാണ്.

Sealed competitive tenders are invited for the supply of good quality bread to Government Ayurveda College Hospital, Kannur, Pariyaram for the year 2016-17. Tenders should be superscribed "Tender for the supply of Bread" and addressed to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram. The approximate quantity of bread to be supplied for one year is 5475 Kg. Rate of bread for one Kilogram should be mentioned in the tender documents. Last date of receipt of Sealed Tender to the undersigned at 3 p.m. on 26-5-2016. Late tenders will not be accepted. Tenders will be opened at 3.30 p.m. on 26-5-2016 in the presence of such of the tenderers or their authorized representatives who may be present at that time.
Estimate Cost—` 3,00,000.
Intending tenderers may on application to the Superintendent, Government Ayurveda College Hospital, Kannur, Pariyaram to obtain the requisite tender forms on which tenders should be submitted. Application for the tender form should be accompanied by a cash remittance of ` 600 + VAT 5% (Total ` 630). Postal charge of ` 50 should be added if the forms needed by post. The price fixed for a form/set of form is not refundable under any circumstances. The tender form are not transferable. Sale of tender forms will be started at 10 a. m. on 5-5-2016 and will be closed at 3 p. m. on 25-5-2016. Demand Draft, Cheques, Postal Orders etc. will not be accepted towards the cost of forms nor will the forms be sent by VPP.

വൈകിക്കിട്ടുന്ന ടെ≠റുകൾ യാതൊരു കാരണവശാലും പരിഗണിക്കുന്നതല്ല. ടെ≠റുകൾ 4-5-2016 വൈകുന്നേരം 3 മണിക്ക് ഹാജരായിട്ടുള്ള കരാറുകാരുടെയോ അവരുടെ അംഗീകൃത ഏജന്റിന്റെയോ സാന്നിദ്ധ്യത്തിൽ തുറക്കുന്നതാണ്.
താൽപര്യമുള്ളവർ ടെ≠ർ ഫോറത്തിനായി "പ്രിൻസിപ്പാൾ, ഗവൺമെന്റ ് വിക്ടോറിയ കോളേജ്, പാലക്കാട്-678 001" എന്ന വിലാസത്തിൽ അപേക്ഷിക്കേ≠താണ്. അപേക്ഷയോടൊപ്പം ടെ≠ർ ഫോറത്തിന്റെ വിലയായി ` 420 [` 400 + 20 (VAT 5%)] അടയ്ക്കേ≠താണ്. ടെ≠ർ ഫോറത്തിന്റെ വില യാതൊരു കാരണവശാലും മടക്കി നൽകുന്നതല്ല. ടെ≠ർ ഫോറം കൈമാറ്റം ചെയ്യുവാൻ പാടുള്ളതല്ല. ടെ≠ർ ഫോറത്തിന്റെ വില്പന 4-5-2016 ഉച്ചയ്ക്ക് 12 മണിക്ക് അവസാനിക്കുന്നതാണ്. ടെ≠ർ ഫോറത്തിന്റെ വില നേരിട്ട് അടയ്ക്കുകയോ മണിയോർഡറായി അയയ്ക്കുകയോ ചെയ്യാം. ഡ്രാഫ്റ്റ,് ചെക്ക്, പോസ്റ്റേജ് സ്റ്റാമ്പുകൾ മുതലായവ സ്വീകരിക്കുകയോ വി. പി. പി.
