No. 1) dated 15-3-2017 issued by the Taluk Supply Officer, Taliparamba, as V.M.Usha in the Secondary School Examination Mark Statement S No. 2006/055912 with Roll No. 4147460 dated 27-5-2006 issued by the Controller of Examinations, Central Board of Higher Secondary Education, Delhi, of my daughter Teena Alex as Usha in the Birth Certificate with Registration No. 54/1991, of my daughter Teena Alexander date of Registration 2-2-1991 issued by the Registrar of Births and Deaths, Naduvil Grama Panchayat, and as Usha V.M. in the Pension Payment Order No. S/034355/1997 (Army) with Regimental No. 01373419 H dated 24-7-1997 issued from the Office of the Chief CDA (Pensions), Allahabad of my husband Alexander V. M, is one and the same person. Hereafter I will be known by the name Usha V.M. only.

സർക്കാർ പ്രസ്സിൽ അച്ചടിച്ച് ഫോൾഡിംഗ്, ഗാതറിംഗ് ചെയ്ത് ലഭ്യമാക്കുന്ന 2018-ലെ സർക്കാർ ഡയറികൾ താഴെ പറയുന്ന വ്യവസ്ഥകൾ പ്രകാരം കെയ്സ് ബൈന്റ ് ചെയ്തു തരേ≠താണ്.
1. 16 × 23 സെ. മീ. സൈസിലുള്ള 120000 ഡയറികൾ, 16 പേജ് വീതമുള്ള ഏകദേശം 26 ഫോറങ്ങൾ അച്ചടിച്ച് ഗാതർ ചെയ്താണ് ഈ കാര്യാലയത്തിൽ നിന്നും ലഭ്യമാക്കുന്നത്.
2. പ്രസ്തുത ഡയറി കവറുകൾക്കായി സർക്കാർ അംഗീകരിച്ചു ലഭ്യമാക്കുന്ന ഡിസൈൻ ഫോർ കളറിൽ (170 gsm) Art paper-ൽ അച്ചടിച്ച് മാറ്റ് ലാമിനേഷനോടുകൂടിയും, ഫോറങ്ങൾ സ്യൂയിംഗ് ചെയ്തും കെയ്സ് ബൈന്റ ് ചെയ്ത് ഡയറിയാക്കി ലഭ്യമാക്കേ≠താണ്.
3. പ്രസ്തുത ജോലികൾക്കാവശ്യമായ കെയ്സ് മേക്കിംഗ്, കെയ്സ് ഇൻ, സെക്ഷൻ സ്യൂയിംഗ് എന്നീ യന്ത്ര സാമഗ്രികൾ തിരുവനന്തപുരം മണ്ണന്തല പ്രസ്സിൽ സ്ഥാപിച്ച് ഡയറിയുടെ കെയ്സ് ബൈന്റിംഗ് ജോലികൾ ചെയ്യേ≠താണ്.
4. ഡയറികൾക്കാവശ്യമായ എൻഡ് പേപ്പർ, കെയ്സ് മേക്കിംഗിനായുള്ള 1.8 mm കനമുള്ള Fusion Board എന്നിവ ഉൾപ്പെടെയുള്ള തുകയാണ് ക്വാട്ട് ചെയ്യേ≠ത്.
5. GST പ്രത്യേകം വ്യ‡മാക്കേ≠താണ്.
6. പൂർത്തിയാക്കി നൽകുന്നതിനുവേ≠ സമയക്രമം ലഭ്യമാക്കേ≠താണ്.
7. പ്രസ്തുത മെഷീനുകൾ പ്രവർത്തിപ്പിക്കുന്നതിനുള്ള വൈദ്യുതി, സ്ഥലസൗകര്യം മുതലായവ അച്ചടി വകുപ്പ് നൽകുന്നതാണ്.

The following is the Select List of Officers approved for Promotion to the Category of Tourist Information Officer, Manager Grade I and Senior Superintendent as prepared by the Departmental Promotion Committee (Lower) in the meeting held on 31-10-2017.
