O. Nediyanga, Sreekandapuram, Taliparamba Taluk, Kannur District, Pin-670 631, holder of S. S. L. C. No. S 591217 with Register No. 530314 of March 2017, PAN Card No. DIPPA3750R issued from Income Tax Department, Govt. of India, have embraced Hindu Religion from Christian-Roman Catholic Community as per Sudhi Certificate No. 8498/2019 dated 5-4-2019 issued by the Arya Samaj Secretary, Arya Samaj Mandir, Ram Mohan Road, Calicut with a new name Arjun T. V. Hereafter I will be a member of Hindu Religion and will be known by the name Arjun T. V. only, and will sign accordingly.
This change will come into effect in all records related to me.
