Kallakatta, Kasaragod District, Pin-671 123, do hereby notify for the information of all authorities concerned and the public that my minor daughter Shalma Smitha Rodrigues, holder of S. S. L. C. No. U 367247 with Register No. 624900 of March 2019 also known as Saima Smitha Rodrigues in the Birth Certificate with Registration No. 6816/2002, date of Registration 8-11-2002 issued by the Health Inspector Gr. I & Registrar of Births & Deaths, Kasaragod Municipality, is one and the same person. Hereafter she will be known by the name Shalma Smitha Rodrigues only.
This change will come into effect in all records related to her.

NOTIFICATION
hereby makes the following amendments in notification No. 9/2019-State Tax dated the 6th July, 2019, published in the Kerala Gazette No. 32, Volume VIII, Part IV, dated the 6th August, 2019, namely:—
In the said notification, in the first paragraph, the following provisos shall be inserted, namely:—
"Provided that the return in FORM GSTR-3B of the said Rules for the month of July 2019 shall be furnished electronically through the common portal on or before the 22nd August 2019:
Provided further that the return in FORM GSTR-3B of the said Rules for the month of July 2019 for registered persons whose principal place of business is in the districts of Idukki, Malappuram, Wayanad and Kozhikode shall be furnished electronically through the common portal, on or before the 20th September, 2019.".
2. This notification shall be deemed to have come into force with effect from the 20th day of August 2019.

LA, Kasargode, also known as Riyas in the Election Identity Card No. JWQ1604883 dated 7-4-2019 issued by the Electoral Registration Officer, Kasaragod Assembly Constituency, and as Riyas Chiminadka in the Passport No. M 5191751 issued on 7-1-2015 by the Passport Officer, Passport Office, Kozhikode is one and the same person. Hereafter I will be known by the name Riyas only and will sign accordingly.
