വിതരണം നടത്തേ≠ സാധനങ്ങളുടെ പട്ടിക, ര≠് ദർഘാസ് ഫോറങ്ങൾ, ദർഘാസിനോടൊപ്പം വെയ്ക്കേ≠ ഉടമ്പടികളുടെ മാതൃക മുതലായവ അതാത് സ്ഥാപനങ്ങളുടെ മേലധകാരികളിൽനിന്നും അവർ നി›യിച്ചിരിക്കുന്ന ദർഘാസ് ഫോറത്തിന്റെ വില സർക്കാരിലേക്ക് അടച്ച് ചലാൻ രശീതും, 1-11-2019-നോ, അതിനുശേഷമോ ലഭിച്ച അസ്സൽ സോൾവൻസി സർട്ടിഫിക്കറ്റും (കുടിക്കടപത്രം സഹിതം) ഹാജരാക്കി ആവശ്യപ്പെട്ടാൽ ലഭിക്കുന്നതാണ്. ദർഘാസ് ഫോറം വാങ്ങുന്ന തിനുള്ള തുക "0210-വൈദ്യസഹായവും പൊതുജനാരോഗ്യവും ജനറൽ-800-മറ്റ് വരവുകൾ-88-പലവകയും" (ഭക്ഷ്യ-ഭക്ഷ്യേതര വസ്തുക്കളുടെ ദർഘാസ് ഫോറത്തിന്റെ വില അടയ്ക്കു ന്നതിന്) എന്ന ഹെഡ് ഓഫ് അക്കൗ≠ിലും, വില്പന നികുതി "0040-വിൽപന നികുതി-102-സംസ്ഥാന വിൽപന നികുതി ആക്ട് 99-നികുതി പിരിവുകൾ" എന്ന ശീർഷക ത്തിലും അടയ്ക്കേ≠താണ്. ഇങ്ങനെ അടയ്ക്കുന്ന പണം യാതൊരു കാരണവശാലും തിരികെ നൽകുന്നതല്ല.

ഓരോ ദർഘാസിനോടൊപ്പം ടെ≠റുകളുടെ പത്ത് ശതമാനം (10%)-ത്തിൽ കുറയാത്ത തുകയ്ക്കുള്ള സോൾവൻസി സർട്ടിഫിക്കറ്റിന്റെ അസ്സലും, ഒരു ശരിപ്പകർപ്പും, കടം വീട്ടാൻ മുതലുള്ളത്, വ്യാപ്തി ഏതുവരെയാണെന്നും, എവിടെയാണ് പ്രസ്തുത കരാറുകാരൻ താമസിക്കുന്നതെന്നുമുള്ള സർട്ടിഫിക്കറ്റും (18 ബി) സ്റ്റോർ പർച്ചേയ്സ് റൂൾ ജനറൽ വിവരങ്ങൾ (സർക്കാർ കത്ത് നം. 36489/എം2/ആ.കു.വ.
