AFFIDAVIT I UMMUL KULUS. K D/o Kidave koya T. K. and Lathi. K, Aged 48 years, residing at Kalkandiyoda, Agathi Island, Lakshadweep do hereby solemnly affirm and state as follows.
1. I am the native of Agathi Island, Lakshadweep and residing at Kalkandiyoda, Agathi Island, Lakshadweep.
2. The passport authority of India were issued a passport bearing No.J2491146 on 28/01/2011 and in the said passport, my name were entered as UMMIL KULUS KALKANDIYODA. Whereas the Correct spelling of my name is UMMUL KULUS. K.
3. It is submitted that in the entire records pertains to me shows my name as UMMUL KULUS. K except in my passport.
4. I swear this affidavit to state that UMMIL KULUS Kalkandiyoda and UMMUL KULUS. K is one and the same person.
All the facts stated above are true to the best of my knowledge and belief

I, SHAIJU DAS Y S/o Yesudas, aged 34 years, residing at Undappara Thadatharikathu Veedu, Thevanpara Tholicode PO, Thiruvananthapuram, Kerala, do hereby solemnly affirm and state as follows:
1. I have born on 02/03/1990 and my birth place and native is Tholicode, Thiruvananthapuram, Kerala.
2. I am married to FASILA, D/o Sayedbuhari Baleliyar, Mannathe Chetta house, Amini Island, Lakshadweep.
3. I have embraced Islam from Christianity and consequently my name has been changed as MUHAMMED ANSAR and presently living at Amini Island, Lakshadweep,
4. I am Wholly renounce, relinquish and abandon the use of my former name of SHAIJU DAS Y and in place thereof do assume from date thereof the name of MUHAMMED ANSAR and so that I may hereafter be called know and distinguish but my former name of SHAIJU DAS Y but by assumed name of MUHAMMED ANSAR.
5. It is found that my name is appearing as SHAIJU DAS Y in some of the records and documents connected/related to me though by name has been changed as Muhammed Ansar.
6. I hereby affirm and state that my name is MUHAMMED ANSAR and I do not wish to call and appear as SHAIJU DAS Y.
7. I am decided to keep my name as MUHAMMED ANSAR in the entire records and documents related to me.
This Affidavit is sworn for the purpose of changing my name as MUHAMMED ANSAR in the official and other records related to me.
