I FATHIMA THALILAPURA D/o Rajab Thirinipura and Hajaromma Thalilapura, aged 61 years, residing at Thalilapura, Kavaratti Island Lakshadweep Pin-682555 do hereby solemnly affirm and state as follows.
1. I have born on 1/08/1954 and my birth place and native is Kavaratti Island, Lakshadweep.
2. It is found that my name is appearing as Pathumma Thalilapura in some of the records and documents connected/related to me and in some of the documents/ records pertains to me, my name is written as Fathima Thalilapura.
3. I hereby affirm and state that my original name is Fathima Thalilapura and I do not wish to call and appear as Pathumma Thalilapura.
4. I am decided to keep my name as Fathima Thalilapura in the entire records and documents related to me.
5. It is difficult to travel with passport and to get grants/privileges from government as my name is appearing differently in various records related to me. Hence, a uniform appearance in the records pertains to me is highly necessary.
This Affidavit is sworn for the purpose of changing my name as Fathima Thalilapura in the entire documents/records related to me.

1. Wholly renounce, relinquish and abandon the use of my former name of Fathima Thalilapura and in place thereof do assume from date thereof the name Fathima Thalilapura and so that I may hereafter be called know and distinguish but my former name of Pathumma Thalilapura but by assumed name of Fathima Thalilapura.
2. For the purpose of evidencing such my determination declare that I shall at all times hereafter in all records, deeds and writing and in all proceedings, dealings and transaction private as well as public and upon all occasions whatsoever use and sign the name of Fathima Thalilapura as my name in place of and in Substitution of my former name of Pathumma Thalilapura.

I, Abdul Samad M, S/o Aboobacker Koya, aged 59 years, residing at Makkashikakkada House, Agatti Island, Union Territory of Lakshadweep- Pin 682553, do hereby solemnly affirm and state as follows:
1. That I am the deponent herein, and I am well aware of the facts stated in this affidavit.
2. That I am the holder of Indian Passport No. M7809093, in which my name is recorded as ABDU SAMAD MAKKASHI instead of ABDUL SAMAD M, and my house name is mentioned as MAKKASHI instead of MAKKASHIKAKKADA. Further, my father's name is entered as ABOOBACKER instead of ABOOBACKER KOYA.
3. That I also hold an Aadhaar Card bearing No. 8267 3774 0235, wherein my name is recorded as ABDUL SAMAD M, my house name as MAKKASHIKAKKADA, and my father's name as ABOOBACKER KOYA.
4. That I also possess a Birth Certificate issued by the Registrar of Births and Deaths, Primary Health Centre, Agatti , dated 24/09/2025, in which my name is recorded as ABDUL SAMAD M, my house name as MAKKASHIKAKKADA, and my father's name as ABOOBACKER KOYA.
5. That due to inadvertence and clerical error, discrepancies have occurred in the spelling of my name, house name, and father's name in the Passport when compared to my Aadhaar Card and Birth Certificate.
6. That the names ''ABDUL SAMAD M'' and ''ABDU SAMAD'' refer to one and the same person, i.e., myself.
7. That the house names "MAKKASHI" and "MAKKASHIKAKKADA" refer to the same house, i.e., that of myself.
8. That the father's names "ABOOBACKER" and "ABOOBACKER KOYA" refer to one and the same person, i.e., my father.
9. That I hereby declare and confirm that my correct name, house name, and father's name are ''ABDUL SAMAD M'', "MAKKASHIKAKKADA", and "ABOOBACKER KOYA" respectively.
THE LAKSHADWEEP GAZETTE 410. That I undertake to use my correct name, house name, and father's name - ABDULSAMAD M, MAKKASHIKAKKADA, and ABOOBACKER KOYA - in all my future records and documents.

2. It is found that name of my elder daughter is wrongly appearing in the Adhaar as Azwa Fathima. The original and true name of my elder daughter is Azwa Sani. Due to the error in the name printed on Adhaar, it is difficult to enrol the details of my child in other official requirements. It is necessary to update Adhaar with true name and address to enable enrolment in official records of educational institutions.
3. I hereby affirm and state that original name of my elder daughter is Azwa Sani and my daughter does not wish to call and appear as Azwa Fathima.
