1. I am a native of Kavaratti Island, Lakshadweep, the passport authority of India was issued passport Bearing No. 3557888 to me on 08/06/2001 and the same was expired on 07/06/2011.
2. I have approached the passport authority of India for renewal of my passport and found error in appearing of name in spelling of my name printed on the expired passport.
3. It is found that my name is appearing as SYEDALI KEEPATAILLAM in the passport issued by the passport authority of India whereas my true and correct name appearing in other records pertains to me as SAYED ALI. K. I (K.I denotes Keepataillam)
4. It is also found that the name of my spouse printed as SULEKHA KANNIPURA instead of printing my spouse's name as SULAIKHABI KANNIPURA. The true name of my spouse is SULAIKHABI KANNIPURA.
5. I hereby affirm and state that my original name is SAYED ALI. K.I and I do not wish to call and appear as SYEDALI.
6. I have decided to keep my name as SAYED ALI. K.I in the entire records and documents related to me.
7. It is difficult to travel with passport and with my identity card as name is appearing differently in both. Hence, a uniform appearance in the records pertains to me highly necessary.

1. That I am a citizen of India by birth and I am working in Defence Accounts Department with Head Quarters at Office of Controller General of Defence Accounts.
Presently posted as AIFA, ICG, IN & MES, Kavaratti, Lakshadweep under administrative control of IFA(WNC) MUMBAI and proforma controller PCDA(NAVY) MUMBAI.
2. That my name has been recorded as "M Ravi Sudhakar" in all my official documents including my service records.
3. That I have now decided to change and expand my name from "M Ravi Sudhakar" to "Ravi Sudhakar Mudigonda", and henceforth I shall be known and identified by my new name only for all official legal, personal, and social purposes.
4. That this change is purely a rearrangement and expansion of my name including expansion of initial M which stands for my surname Mudigonda and does not involve any change in my identity or any fraudulent intent.
5. That I am the father or Mudigonda Lakshmi Sravya, who studied at Army Public School, Ghorpuri, Pune- 411001, under the Central Board of Secondary Education (CBSE).
6. That my daughter Mudigonda Lakshmi Sravya passed Class X in 2019 under CBSE Board with Roll No. 4194001, and Class XII in 2021 under CBSE Board with Roll No.15620389.
7. That in her CBSE records, my name has been recorded as "M Ravi Sudhakar", and I hereby affirm that "M. Ravi Sudhakar" and "Ravi Sudhakar Mudigonda" refer to the same individual myself, her father.
8. That my Aadhaar Card bearing No.3111 3098 5141 has already been updated and reflects my name as "Ravi Sudhakar Mudigonda".
9. That I am executing this affidavit to declare my change of name and to request the concerned authorities to update and correct my name in:
