1. I am born on 30/09/1955 and my birth place and native is Kadmath Island, Lakshadweep.

2. It is found that my name is appearing as SOUDABI PATHADA MANZIL in the Adhaar Card issued by the Unique identifi cation authority of India whereas my true and correct name appearing in other records pertains to me as SAWDHABI PATHADA.

3. I hereby affi rm and state that my original name is SAWDHABI PATHADA and I do not wish to call and appear as SOUDABI PATHADA MANZIL.

4. I have decided to keep my name as SAWDHABI PATHADA in the entire records and documents related to me.
